Continuity mistake: When Tracy and Bond are talking in the casino, and Bond tells her to stay on 5, she has her left hand resting against her chin. As she says "people who want to stay alive play it safe" she moves her hand down to the table, but then when the camera cuts back to Bond the reverse shot of her shows her hand still by her chin, and then she moves it down again. Revealing mistake: When Bond and Che (the big black guy) are fighting, Che lands on a rug behind Bond. As he gets up again one can see a brown mat underneath the rug. It's very noticeable by the way it moves as Che pushes himself up. (00:13:55)

Continuity mistake: When Bond and Che are fighting, Che knocks out one of the white wooden bars behind Bond. However, in the next shot, two bars are missing on the upper half of the wooden structure, one on the bottom half. Seconds later, just before Bond throws Che into it, the bottom half of the structure is complete again, and only one bar is missing on the upper half. (00:14:15)

Continuity mistake: After Bond has knocked out Che for good, Che's position changes between shots. Especially his legs are in a different position in the wide-angle shots to the shot in which Bond has walked up to him. (00:14:40)

Continuity mistake: As Tracy holds the gun on Bond, as she walks toward the camera she lifts the gun closer to her face, so that it is pointed up. The camera shifts back to Bond and the gun is suddenly pointed at him again. (00:16:30)

Continuity mistake: When Bond is forced to get into the car outside the hotel, the car's roof is down and secured with a buttoned-down cover. However, during the ride the roof suddenly closes automatically although the cover was never taken off. (00:19:40)

Audio problem: When Bond is talking to Draco at his house, Draco is telling Bond about his daughter Tracy. As he sits down on the sofa next to Bond, brushing cigarette ash off his suit jacket, he is clearly and rather badly dubbed. (00:24:40)

Continuity mistake: When Bond is finishing up copying the documents in Gumbold's office, with Gumbold about to return via the elevator, a clock tower is shown at 13:59. Without the shot changing, the hands immediately move to 14:00, but some pigeons nesting on a ledge below abruptly change position. (00:42:58)

Revealing mistake: When the white race car with the number 6 is turned over there is a closeup shot of the driver obviously hanging head down in his seat. If you look closer you can see the spectators in the background also upside down. (01:47:20)

Continuity mistake: At the start of the scene where Bond is helping with the assault on Piz Gloria, look at Tracy's hand just after the bald guard presses it down on the object to disarm her. You can see blood, but a couple of minutes later, when she kills him, there is no blood to be seen. (02:01:25 - 02:02:40)

Continuity mistake: When Tracy is battling the balding guard during the climax, she throws him into a tall, ringed guard rail in a way that ends up with one of the circular sections stuck on his head like a crown. When he finishes falling down the stairs, this circular section disappears. (02:02:00)

Other mistake: As the assault on Piz Gloria begins and the helicopters approach the facility, on the right of the screen one seems to be hovering near the helipad as another passes behind it. The helicopter in front fades to a slightly different position as the one passing it disappears and reappears several meters ahead. On the left of the screen the third helicopter appears out of thin air. (This was obviously caused by a dissolve effect, but since the sound effects are constant there doesn't seem to be any indication that a visible edit or time shift was intended for this scene, so it's a mistake). Revealing mistake: While the lab explodes, Blofeld jumps a ditch followed by Bond. Bond's stunt is noticeable because of his longer and light brown coloured hair, as opposed to Bond's very dark colour. (02:11:40)

Other mistake: In the scene when the helicopters arrive over the research center, look at the heli in the bottom right - it just fades out, while one on the left materialises, and the one in the middle changes position. Notice that after all the helicopters have changed place, you can see the helicopter that faded out appear on the helipad. (02:25:25)

Deliberate mistake: Blofeld doesn't recognize James Bond in this film, even though they met face-to-face in the previous movie, "You Only Live Twice." There is a production-related reason for this. Ian Fleming wrote "On Her Majesty's Secret Service" in 1963 (in which Bond and Blofeld met for the first time), and he wrote "You Only Live Twice" in 1964. However, "You Only Live Twice" was adapted for film first (in 1967), and "On Her Majesty's Secret Service" was adapted afterward (in 1969). Because the 1969 film was so faithful to its source material, Blofeld and Bond are basically meeting for the first time... again. The producers were aware of this continuity problem and intended to have James Bond undergo plastic surgery for "On Her Majesty's Secret Service" (which would conveniently explain Blofeld not recognizing him, as well as the fact that Sean Connery had been replaced by George Lazenby in the lead role). But the plastic surgery idea was discarded in faithfulness to the novel, resulting in a glaring continuity problem between the 1967 and 1969 films.

Audio problem: In the opening scene, Bond sees Tracy in the ocean and drives his Aston Martin onto the beach to rescue her. When he stops, we hear screeching tyres, but he is driving on sand.

Revealing mistake: When Bond throws a Blofeld henchman off a cliff near Piz Gloria in the Alps, you can see that it is a dummy, as its arm swings round and wraps around the whole dummy.
