Continuity mistake: In the beginning of the movie when Neo gets the FedEx envelope, he is shown at his desk opening the envelope and then pouring it out. As he is pouring the phone out, with the green packing label on the top, the shot cuts "seamlessly" to a close up of the phone being poured out the rest of the way. However, the envelope is now flipped over. The FedEx logo is now on top (the packing label is facing down).
Continuity mistake: When Neo is brought out of the building, you clearly see the police officer push the car door open and turn his head to the rear of the car, yet in the next cut where we are viewing the scene from the reflection in Trinity's side view mirror his head is facing forward again.
Continuity mistake: On the skyscraper, when Neo and Trinity kill all of the soldiers, Agent Brown comes to the roof. Neo immediately draws his guns and fires until both of his clips are empty. After that, Neo drops his guns, just as Agent Brown aims at him. During Bullet-Time there are no guns on the floor. When he hits the ground, then the guns are left and right of his legs.
Continuity mistake: In the interrogation room. Mr. Smith has a green file with him with Neo data accumulated. Smith opens the file and leafs through a few sheets, without removing any, but in subsequent shots we can see that some sheets have been removed from the file and put on the left.
Continuity mistake: The first time Neo comes out of the Loading Program, and is about to throw up, it shows you a shot from his eyes and the floor is clear of anything, and then it does a shot of him throwing up and he does so on a bunch of wires. The angle changes too - the POV shows all the lines in the grate at about 45 degrees, but when he throws up, they're all horizontal in relation to him.
Revealing mistake: During the fight scene between Neo and the agent in the subway station, after Neo incapacitates the agent and flips out of the way of the oncoming train, you can see where the wire harness connected to his back. You can also see one of the wires visible by his wrist.
Other mistake: When Trinity confronts the agent on the rooftop and says, "Dodge this", she points her gun to temple of the agent while his head is turned to her. He glances back, but never fully turns his head. The next scene shows him being shot facing Trinity...with a shot in middle of his forehead.
Continuity mistake: In the subway fight scene, the shot where the agent is holding Neo down while the train is coming, you can see the Agent's tie in front of his arms, in the shot just as Neo is getting ready to jump with the agent you can see the agent's tie neatly pinned back with a tie clip.
Continuity mistake: When Neo and Morpheus are in the hallway to see The Oracle, the shot of Neo's hand on the knob shows Morpheus with his sunglasses on. Morpheus is in shot the whole time when the door's opened, and he doesn't move an inch, but in the front shot as he walks through the door, his sunglasses have suddenly been removed.
Other mistake: When Morpheus is being interrogated by the agent alone, the camera switches from behind Morpheus, to just forward of him and back a few times. If you look at the agent's hand on Morpheus' head, you can see that his fingers switch from behind his ear to both sides of his ears as the camera changes.
Continuity mistake: When Neo gets the phone from the delivery guy, he goes to open the package as the guy is leaving. You can see that when the guy leaves the shot Neo is about to reach in to get the phone, but when the delivery guy is gone completely, Neo is only moving his hand over to start opening the package.
Revealing mistake: When Trinity's being chased by the cops in the first scene and she makes her first jump from one roof to another when being chased by the cops. One of the cops that is chasing her doesn't quite make a complete jump to the other roof. During his failed attempt when he hits the lip of the roof of the adjacent building it makes the whole building shake.
Continuity mistake: After filling a room full of holes with the machine gun from the helicopter, the Agents abandon their current bodies and take control of ones that were standing outside the door to the room Morpheus was held in. Where were these guys the whole time the gunfire was happening in the next room? The Agents were standing in relaxed positions as soon as they took control. Wake up guys.
Visible crew/equipment: When Neo is at the office, while in his cubicle, he gets an express delivery. As he signs for it you can see another arm laying on the desk behind him. In the next shot he hands back the clipboard, you see it is only him and the delivery man. [Only visible in 4:3 format.]
Deliberate mistake: When Neo and Morpheus are in the construct place (where it is all white), you can see that Morpheus' glasses are a matte grey and then a second later they are totally reflective. [The shot where Morpheus has flat black glasses was shot with a steadycam. A cameraman using a special camera strapped to his body walks in a snaking path from behind Neo, to behind Morpheus. He passes in front of Morpheus at least once. His glasses were painted flat black to prevent the reflection of the steadycam operator showing up on film. At the end of the shot, you can see the back side of the lens. While doing this scene and saying his dialogue, Laurence Fishburne couldn't see anything.]
Continuity mistake: This is a small one but easy to see. When Neo is looking at his computer showing messages about the matrix and the white rabbit, there is a shot of Neo asking "Who is it?"; then we see a shot of the black screen, and finally Neo face again, but with a different hairstyle.
Continuity mistake: Throughout the "lobby scene" the walls and pillars of the lobby are shown being torn apart by bullets. At the end of the scene, Neo and Trinity proceed into the elevator and the overall damage is apparent. They set the bomb, get out of the elevator and the bomb and elevator drop. When the explosion is exiting the elevator shaft, the lobby is spotless... no SWAT men, no guns, and perfect pillars and walls, not a bullet hole in sight. It could be said that the dead guards are on the sides of the room, invisible from the camera's angle... but what about the last guy Neo kills right in front of the elevator?

Factual error: When Neo is shooting thousands of bullets out of the Gatling Minigun from the helicopter, there is a shot of "cases" and links falling from the machine gun onto the helicopter floor. Only those "cases" are complete rounds, bullet and all (or possibly blanks) instead of just empty, spent cases.
Other mistake: When Neo is being chased by the three agents through the outdoor marketplace, pay attention to the shot of the guy being picked up by an agent and flung through the air. There is a woman wearing a red sweater to to the slight left of the agent. She does not flinch, even though the guy in front of her has just been thrown 15 feet into the air and into a produce stand. She just stops for a second to let the guy fly past her, and she then simply resumes her walk. Other people around her react...
Continuity mistake: When on the rooftop and the agent and Neo are shooting at each other Neo runs out of bullets and places the guns in front of him he starts avoiding the bullets. The camera angle changes and the guns disappear. When the camera angle goes back the guns are there and he reloads them.
Factual error: During the lobby shootout, Neo pulls out a pair of VZ-61 Skorpion machine pistols and runs down the lobby blowing away the guards. While he is running, we see the blank shells falling to the ground beneath him. However, the shells are 5.56mm NATO, which is a cartridge that is too long to fit in this gun.
Visible crew/equipment: When Neo and Smith have a fight at the train station, before Smith drags Neo by the legs and tosses him onto the train track, he punches Neo into another wall. Just after he does this, a red piece of equipment is visible to the left of the screen, being pulled quickly out of shot.
Continuity mistake: When Neo unloads two 9mm Berettas at an agent who dodges all the shots, right after he runs out of ammo, the camera is facing Neo dead on and the recoil slides on the pistols are closed. The next shot where he drops them and says, "Trinity... help." the slides are locked back with the ejection port open.
Continuity mistake: When Neo drops the FedEx envelope on his desk after he pulls out the cell phone from the delivered package, it lands right next to a keyboard on top of a cable/wire. When the phone immediately rings and he answers the call, in the wider shot showing the whole cubicle with Neo still in his seat, the envelope is now resting partway on top of the keyboard. Then when he stoops next to his desk after nervously crouching about in his own cubicle wondering to Morpheus what the unknown group of men who is coming to his office wants from him, the overhead shot shows the envelope now closer to the edge of the desk (bottom of the screen), well clear of the keyboard. Yet in another shot from outside the cubicle just a little later, as he makes a dash out of his office according to Morpheus' instructions, the envelope is back resting on the keyboard. Those FedEx packages must really move. Also, what's the deal with the cable that's unplugged on his desk?
Revealing mistake: When Neo shoots the guy with the newspaper at the beginning of the lobby scene it is obviously a prop. It is very thick paper stuck together almost like cardboard because the individual pages are not seen where it rips. Also it would be near impossible to split a newspaper like that even though it does take a few bullets.
Other mistake: When Morpheus makes his escape, Agent Smith shoots through the walls with a noticeable time between shots. However when you see the bullets in motion (the 'bullet time' sequence), they're so close together in time that Morpheus doesn't appear to move in the interval between shots. Even if these are supposed to be later shots they could not be fired so close together either by the agents or by the guns used. The first two bullets are some distance behind Morpheus, corresponding to the change of direction of the shots fired by agent Smith to track Morpheus' movement - this also doesn't fit with the shots seen in the 'bullet time' sequence.
Continuity mistake: Near the end, after Neo steals the cell phone, as he's running away, you first see the cell phone owner from the front, waving his right arm and shouting. The next camera angle shows the owner from behind waving his right arm, but he has obviously already been inhabited by the agent (the suit colour's the giveaway). The next cut has the "Agent inhabiting body" sound effect, and he turns around to begin chasing Neo.
Deliberate mistake: In the rooftop fight scene where Neo unloads 2 pistols at an agent and misses, you can hear the guns firing from behind, followed by 2 clicks suggesting that the clips are empty. Neo's guns are both semi-automatics and would therefore not click when empty as the slides would be locked back after the final shot. The click sounds were obviously added later to indicate to viewers that his clips were empty.
Continuity mistake: When Morpheus fights Neo in the training program, Neo is pushed backwards and slides along the floor. In the close-up, his head stops level with one of the black stripes on the floor, so that his eyes are looking at it. In the wide shot in which Morpheus jumps in slow motion through the air, Neo's head is past the line so that it is level with his chest, as if he has slid forward further, despite having stopped previously. In the close-up, the line is again level with his eyes, but when we see Morpheus' knees crashing into the floor, Neo has again moved forwards such that his head is no longer level with the line.
Continuity mistake: When Morpheus shows Neo the agent training simulation, the girl in red turns into an agent. When they pause the simulation you can see the reflection of Neo with a gun pointed at his head in Morpheus' glasses. When you look in one shot there is a visible white line/mark (scenery?) between the gun and Neo, in the next shot it is gone and then right at the end in the final shot it's back again.
Continuity mistake: When cops are in the room with Trinity at the beginning, just before she attacks the first cop, the chair she sat in is really close to the table, yet when she is in the air its a lot further away from the table. Also, the cop holds in his left hand a torch, which disappears when Trinity goes up for the kick.
Revealing mistake: When Neo and Agent Smith are fighting on the subway tracks, Neo leaps up to avoid being hit by the train. While the shot is in slow motion, you can actually see the wire supporting Keanu Reeves (just look at Neo's hands grabbing them). The wire becomes momentarily visible right when he is reaching the highest point of this jump.
Continuity mistake: During the rooftop scene check out the tiling situation. When Neo takes two bullets, he falls to the ground. He lies mainly inside one tile, even the left gun fits in. (And it's on a vertical line) His top hand lies just on the line. Now, watch again when the agent is dead. All of the sudden he's in-between two tiles, and the gun is now on a horizontal line, but not a vertical.
Continuity mistake: When Agent Smith is seen the first time, he is talking to the old cop outside, then begins to walk away and the other agent follows. The cop turns his head to continue to look at the other agent when he is walking away or past him, yet in the following shot the cop has his head turned, but the other agent is still in sight and is just barely walking past him.
Continuity mistake: At the beginning of the film, when Neo is having the conversation with his boss about his arrival time at the office, when the camera is on Neo, his hands are behind his back, when the is on his boss (showing Neo's back), Neo's hands are in front of him. They're all instant cuts - no time for him to move.
Continuity mistake: Near the middle of the film when Morpheus directs Tank to load the Jump Program note where Morpheus and Neo land on roof of building. Aerial view shows them landing some distance from the building but when the camera cuts to the first close up they are standing right next to the building.
Factual error: When Agent Brown shoots at Neo on the skyscraper, the last shot scrapes Neo in the knee and on his shoulder. This would cause both shots to be vertical when standing up. However, when Neo stands up and indicates to the scrape, the shoulder scrape is horizontal - not possible given the trajectory Agent Brown is firing from.
Continuity mistake: After the helicopter smashes into the building and Neo saves Trinity, they look at each other and you can see smoke from the building. After that, Morpheus is talking to Neo. You can see several shots of him and Neo talking and behind them, the building and its windows are still intact.
Other mistake: When Neo shoots with the chopper's machine gun the window smashes from a single point in the middle, shattering two panes. Water then starts splashing up from bullet impacts right at the base of the window, but the glass doesn't smash anywhere around there - it could only work like that if Neo was shooting down at an incredibly sharp angle, which he obviously isn't.
Continuity mistake: Near the middle of the film when we see Morpheus and Neo have entered the Matrix in a scene that begins at a city intersection with a "Walk/Don't Walk" Traffic Sign. Just as the cross the street, Morpheus is speaking to Neo, the next shot looks over Neo's shoulder back towards the intersection, it is no longer visible, as though they have traveled farther down the block than physically possible in the time allotted.
Continuity mistake: During the fight scene in the train station between Agent Smith and Neo, after a lot of fighting Neo is thrown back and throws up blood. He then gets up and wipes it off with the back of his hand. Straight after that, it isn't visible anymore. It's completely vanished.
Continuity mistake: Near the end of the film we see Neo in the chopper firing the mini gun through the window at the agents. In one particular shot note the metal chair pulled out from the desk which falls over after being shot at, then a few shots later is suddenly back upright again.
Continuity mistake: In the first scene in which Neo appears, we see him from above, sleeping in front of his computer. The keyboard he is using is a Microsoft Natural Keyboard Pro with the atypical curved key configuration. When messages begin appearing on his screen, he tries to stop them by hitting various keys, but the closeup of the keys show keys from a standard keyboard that are clearly not the keys on a Natural Keyboard Pro.
Continuity mistake: In the fight in the toilet between Morpheus and Agent Smith, they're on the floor and Smith has his glasses on. Smith prises Morpheus' hands off him, punches him back against the wall, stands up in a menacing way, and his glasses aren't on any more. He wasn't hit at any point in that brief exchange, so where have his sunglasses disappeared to? [Only in the UK version - the BBFC's got a thing against headbutts, so cut several out from the film. In this case it caused Smith's sunglasses to disappear for no reason.]
Continuity mistake: When Neo is in the helicopter firing the automatic weapon into the building he is moving the gun horizontally. The bullets strike vertically, though, starting from the floor near the window and traveling upwards towards the back wall of the room. There are two shots that show this.
Continuity mistake: In the scene right after Neo steals a man's cellular phone, we see him running down an alley with the back of his shirt untucked from his pants. After cutting back from the agent who's chasing him, we see that Neo's shirt has miraculously tucked itself back into his pants.
Continuity mistake: When the guard asks Neo to remove any metallic items after he sets off the metal detector, Neo's coat is open, and he's just got a T-shirt on, with no guns strapped to him. In the closeup, all the guns are visible. Most are down the side, but we should have seen the pistols in his belt from the earlier angle. If nothing else, with his coat open as much as it was, the guard should have spotted the weapons even before he opened it further...
Continuity mistake: When Tank comes into Neo's room and talks to him, just before his training, note that there is a rip in Tank's shirt near his right shoulder. Near the end of the scene, the shot changes to Neo, then when it cuts back to Tank, there's no rip. When it cuts to Neo and back to Tank again, the rip is there.
Revealing mistake: In the middle of the film when we see Cypher talking to Trinity on the phone, at the end of their conversation we see Tank with the big electric gun, when he fires it, pause and use frame by frame or slow motion and you can see its not Cypher (Joe Pantoliano) but his stunt double, this can be seen because the brightness of the gun causes his face to be bright so you can see the stunt double.
Continuity mistake: Near the end of the movie after agent Smith shoots Neo. Trinity is whispering to Neo, as she leans toward him there is a small strand of hair on Trinity's right temple slowly falling down on her face. The shot changes to Neo's face and back to Trinity where her hair is now perfectly in place and the strand is gone.
Continuity mistake: In the beginning of the movie, where Trinity runs from the agents, she jumps towards a window. She turns slowly in the air, eventually facing upwards when she crashes through the window, no longer turning. From the inside shot, however, she comes through it face down (so she can roll forwards down the stairs).
Continuity mistake: When Morpheus and Neo are fighting in the training simulation, just after Morpheus says, "You think that's air you're breathing now?" there is a pause before he prompts Neo, "Again." The shot goes to others watching on the monitors, notice Dozer looks to his left in the direction of Mouse. In the next shot the camera cuts to Cypher and Trinity (who are sitting behind Mouse) but Dozer's head is now facing the opposite direction it should be.
Continuity mistake: When Trinity and Neo have rescued Morpheus and are flying through the air, Neo drops Morpheus then Neo lands, but Neo is halfway on the rooftop, yet when he grabs hold of the rope and is dragged along the rooftop, he is sliding almost the entire length of the rooftop.
Continuity mistake: When Agent Smith is fighting with Neo in the subway, Neo's back is against a pillar. The area of the pillar which Agent Smith punches away with the first punch is visible before Neo ducks, and then he punches it away. You have to look carefully, because it only appears for a very short period of time.
Other mistake: In the bullet-time sequence in the subway station the lighting on Neo and Agent Smith doesn't change even as they spin round each other, principally because they were in a green screen chamber with even lighting to shoot the movie. It sticks out because the lighting on the CGI set (for the bullet-time shot) is pretty dim with light in specific areas.
Continuity mistake: In the Oracle's house there is something wrong with the glasses in the cupboard behind Neo. When he's looking at the children, there are three small glasses in front, a big one behind and something else beside them. In a shot from below when the boy hands the spoon to Neo, the big glass has now moved to the left of the small ones.
Continuity mistake: When Trinity leaves the matrix at the start through the phone booth, the garbage truck driven by the agents rams the phone booth and punches a big hole in the brick wall. When the truck reverses there is no visible damage to the truck, not even a broken headlight, even though it has just crushed a phone booth and a brick wall.
Continuity mistake: Near the end when Neo runs and jumps into Agent Smith's body, the other two agents are shown standing at the end of the hallway. If you look closely at the shot of the agents after Agent Smith explodes, the tie clip of the agent on the left side of the screen is angled diagonally across his tie. Neo flexes the walls, and in the next shot of the agents the tie clip has returned to where it should be - horizontally across the tie.
Continuity mistake: As Neo awakens inside his "goo tube", a few from overheard shows him raising his arms against the covering of his tank. The shot changes to one from the front and suddenly Neo is in a sitting position and raising his arms up as he breaks the covering. When Neo was first raising his arms, he was laying on his back.
Character mistake: When Morpheus is explaining to Neo the reason the Matrix exists, he states that a human being gives off 25,000 BTUs of body heat to help power it - this is just not true. Even human beings that are running a marathon can't give off that much heat, and the humans in the Matrix are all lying down, not moving.

Continuity mistake: When the agent is running after Trinity, Trinity gets to the end of the roof and runs on. Notice that the end of the roof is red, or a rusty like colour, but when when the agent starts shooting at her, it's turned from red back to probably its original colour of silver.
Deliberate mistake: When Neo is in the helicopter shooting the agents through the window, watch as one of the agents shoots. He obviously doesn't pull the trigger, because he doesn't move his finger, and there is no recoil, nor a shell ejected. But yet there is a muzzle flash.

Continuity mistake: When Neo steals the guy's mobile phone there is a shot looking back at the man (agent Smith) waving his arm . On the pavement there a few people going both ways. When the close up of Smith happens there are loads of people going left to right and more people on the right side of the screen.
Continuity mistake: When Neo and Morpheus are inside the hotel room and Morpheus is giving the offer to Neo, the reflection in either part of his shades shows that he is holding his hands away from each other. Yet, when Neo goes to pick up the red pill, they are close together, then we see them further apart from the reflection inside the shades.
Factual error: In the "Deja Vu" sequence, when Mouse opens the box containing his Auto Shotguns, they are stored with a few belts of .50-caliber BMG ammunition, which would be useless in a shotgun. Also, despite firing about 20 or more 12-gauge shotgun blasts at the SWATs, he fails to hit anyone, even though that equates to about 240 flying lead pellets.
Factual error: When Neo was getting shot outside room 303, Smith fires 10 rounds into him. More if you count every time Neo is shaking in the seat as additional shots. The Desert Eagles in the movie are chambered for .50 AE. You can tell this because the barrel isn't fluted. This means it has a 7 round magazine. Agent Smith runs out of ammo in the subway, so it's not like he can hack his gun because of his software.
Visible crew/equipment: When Neo and Morpheus are in the training program with the girl in the red dress, after Morpheus has paused the program you can see a camera and a black fabric cover around it reflected off Morpheus' sunglasses for a considerable amount of time. Noticeably in the shot considerably close to his face while talking.

Other mistake: They are presumably in the US - trying to make it look that way, anyway. Cars driving on the right, speaking English etc. However the filming location was overseas, revealed by the elevator in the lobby scene. When they hit the button, the plate refers to the elevator as a lift.

Revealing mistake: When Neo stops the bullets being shot at him in mid air and they fall, you can see that there are not grooves in the bullets. As a bullet passes through a barrel, it picks up the impressions of the lands and grooves of the barrel. Since the Matrix is a simulation of our world, including the involved physics, the lack of rifling grooves is an oversight by the special effects team.
