Visible crew/equipment: When Andy goes upstairs after being introduced into the Simpson's home, he walks down the hallway and then backtracks to Kyle's bedroom door and opens it. As the shot pans closer in as he opens the door, the camera and its operator are shadowed on the door. (00:12:05)
Audio problem: When Mattson throws Chucky into the back of the trunk, he tries to close the trunk. However, the trunk won't close because it hits Chucky. You can barely hear any sound when the trunk bumps into Chucky. When it cuts to Mattson trying again, you can hear a bumping sound. (00:14:35)
Continuity mistake: When Kyle manages to stop the truck, we see Andy running away with Chucky on his shoulders. It cuts to Kyle and the driver for a second before Kyle runs after Andy. In the next shot, we see Kyle running but we don't see Andy. Andy could not have run so fast that we wouldn't be able to see him anymore. (01:02:05)
Other mistake: At the beginning, we see the workers attaching Chuck's head to his body. But there's no place for the wires that would operate the dolls, seeing as how the workers believe he's just a normal doll.
Plot hole: In the factory we see the machine that adds the hair to the Dolls it "punches" the doll at standing level. Andy hits the reverse button and they watch a doll go backwards. A few moments later Chucky appears on a cart and now the machine "punches" Chucky to the cart. 2 Errors -1 How did Andy and Kyle not see Chucky on the cart right in front of them. 2. What made the machine go low enough to stick Chucky to the cart.
Continuity mistake: The front part of Kyle's red Ford is damaged when she is trying to get rid of Chucky and smashes into a pole. However, when she runs to her car so that she can chase the Chicago Sun-Times truck, in the overhead shot of the front of the car as she reverses, the car is intact. (00:56:25 - 01:00:25)
