Continuity mistake: When Tony shoots Manny after he finds out that he has been with Gina, she runs to hold his bloody body, in turn getting blood on her robe. A few scenes later, when Gina comes into Tony's office, wearing the same robe, it is perfectly clean and there is no sign of any blood.

Continuity mistake: When Tony is in the Babylon Club watching the performer dressed in a clown suit, the assassins shoot everyone, including the clown. When the clown is first shot, his hat falls to the ground (but the camera cuts away). After a few seconds later, we see the clown again falling, but his hat is back on top of his head.
Continuity mistake: In the final scene where Tony is killed, he is shot, then falls through a banister. When he first falls through the banister both halves are pushed outwards. The next shot is from below looking upwards and shows one half of the banister coming outwards and the other going inwards. Then it goes back to the shot from above and both halves are back outwards.
Continuity mistake: In the scene at the beach/pool where Manny approaches the blonde in the blue bikini, if you watch the area of the pool behind where the blonde is seated on a lounge chair, you will see several people in the pool. Immediately after Manny sticks his tongue out at her and the camera pans back to her as she slaps him, if you look at the same area of the pool, there is absolutely no one there anymore. This was only a matter of seconds after seeing everyone there.
Continuity mistake: Tony tells Nick to call Lopez at exactly 3:00. The scene goes on with Lopez and Bernstein dying when they ask Ernie if he wants a job. The clock says 3:03 but, from the time the call was made at 3am to the next time we see the clock next to Ernie, 5 min and 25 sec have passed.

Continuity mistake: Just before Michele throws the drink in Al's face, when they keep cutting from her to the others, her body position is different each time. Sometimes, she is leaning forward, with her left hand on the drink, other times she is leaning back, with her right hand up holding out her cigarette. When the camera is on Al and you just see her in the frame, her body is still, inconsistent with her body position changes.
Continuity mistake: In the scene when Tony goes visit his mother, Gina hugs him and you can appreciate her long and neat nails. Right after the hug, Tony gives her a present that she opens, and then the camera shows only the jewel and (supposedly) Gina's hands touching it. These hands have really short and not so neat nails. You can see Gina's long nails again at the end of the scene when she holds Tony again outside by the car.
Continuity mistake: In the scene where they are having dinner at the Babylon Club, the waiter brings them champagne and when they all raise their glasses to toast, Tony has a cigar in his mouth. The camera cuts away and then cuts back to Tony about to take a drink of champagne and the cigar is now gone. There was not enough time to take out the cigar. A second later the cigar is back.
Continuity mistake: In the scenes after Omar is killed in the helicopter, Sosa and Montana are talking business gangster style. Montana's body shifts positions all the time as the cam cuts, one time he is standing with his arms akimbo, next he is bringing his hands to his hips, and all the time his body's angle is shifting.
Continuity mistake: In the motel chainsaw scene, before the Colombians pull out their guns, there are several food containers, a bag of cookies and a soda can lined up neatly on a table, seen in multiple shots. After the skirmish, Hector pulls the chainsaw out of its box and Tony is leaning against the table with the items now all scattered in different positions. A few minutes later, Manny shoots his way in. You can see the table in multiple shots and the items are back in their original positions.
Continuity mistake: Angel is killed in the tub in the motel, but his body is not in there when shots of the tub are seen. Also, if the body were still in there, Tony and the other man would not be able to stand or move back and forth as easily as they did as they would be slipping and stepping on body parts.
