Continuity mistake: When Jack is strangling Donald at the end of the film, take a look at the blood that is on Donald's head. It disappears for a while and then it's there again. (01:22:20)

Continuity mistake: When Cole and Campbell get a call over their radio about a suicide, Cole picks the receiver up with his left hand, but in the next shot it is in his right hand. (00:05:00)

Continuity mistake: At the end of the film we see Jack have a fight with Donald. Then Jack says " If thats the best you've got, I'm just going to have to kill you". Then Jack grabs Donald around the neck with both hands, but when the camera changes to behind Donald, Jacks hands are not around Donald's neck. (01:22:20)

Continuity mistake: When Cole is at his wife's funeral, his new girlfriend/wife is standing by his side with her hand on Cole's son's right shoulder, but in the following shot both hands are on the son's shoulders. (00:28:35)

Continuity mistake: When Campbell first walks into his office to find Cole sitting there, he says his line, and in this shot his arms are in front of him holding a folder, then in the following shot his right hand is now on the desk. (00:02:55)

Continuity mistake: At the end of the picture, why are there fire trucks there with fire hoses laying in the street? There was no fire, just shootings.

Continuity mistake: At the end of the film, when the bad guy is trying to escape by going out the front door of the motel, if you slow it down, you can see that the door is slightly open, but in the following shot there is no gap in the door. (01:19:45)

Revealing mistake: After Seagal and the student jump off the window they land on the floor and for a brief but obvious moment one can notice it's not Seagal but his stunt double.

Audio problem: When Cole says "Boring" during the end fight scene, his mouth does not move.

Continuity mistake: When the two Russian mafia guys pretending to be Internal Affairs come to take Seagal away from his home, the windscreen wipers are vertical when they leave the car. On returning to the car, the wipers are now in the normal parked position.

Other mistake: At the scene of Coles' ex wife's murder, the police car nearest the footpath is impossibly parked. There is a lamppost inches from the front of the car and a tree inches from the rear. Did it parachute in?

Trivia: Steven Seagal had some sort-of religious "breakthrough" before filming began and decided he didn't want to kill any villains in the movie. This caused some friction on the first day, when he was set to film a scene where he killed Stephen Tobolowsky. Tobolowsky, knowing how spiritual Seagal was, ended up convincing Seagal that killing his character was the right thing to do, as it would allow his character to be redeemed and reborn in a new body through reincarnation. The scene was filmed as it was scripted. Later, however, Tobolowsky got a call from the director, who informed him that Seagal kept improvising lines implying that Tobolowsky survived. Tobolowsky ended up having to come back out and record some wonky dialog of his character screaming "Finish me off!" to imply he survived and appease Seagal. In the end, the lines weren't used and Seagal's improvisations were simply cut out, so Tobolowsky is killed off as was originally planned.
