Other mistake: The old man executed in District 11 was shot in the head from behind. However, when Katniss looks out the window from above there are no blood stains on the floor, nor is there a gunshot wound to the head. (00:20:50 - 00:22:30)

Other mistake: While Katniss and Johanna are unreeling the wire on the way to the beach, it snags (or is deliberately pulled) behind a rock and Katniss tugs on it till it snaps. The wire was coming off a spool and becoming caught up would have no effect on how the wire unreeled, because the spool would keep on turning regardless. In fact, a taut wire behind her would be what she wanted. (02:05:35)

Continuity mistake: Prim's cat from the first movie is black and white. In this movie, it is orange. (00:04:30)

Continuity mistake: Just as Katniss is preparing to go on stage with Caesar, Joanne speaks to her backstage just as Katniss starts walking out. However from the view of the audience, Joanna is up on the platform with the other tributes. (01:10:30)

Audio problem: When the group is discussing why the Careers aren't on the beach, Beetee asks what happens at midnight and Katniss replies that the lightning strikes the tree. Beetee then says "Here's what I propose", but neither his mouth or jaw moves along with the line. (01:58:25)

Continuity mistake: In the training centre, after Brutus throws the spear and after Katniss has shot arrows at the enemies made of orange light (when the shot pans out) the weapons they used have disappeared and are not on the ground. (00:59:10)

Suggested correction: That's not a mistake but intentionally done to show the technology. It's not explained how it works, but the weapons disappear as it hits the targets.

Continuity mistake: When they bring the old man from District 11 to the front of the crowd to be shot down, after Katniss is dragged back inside the building, the old man turns his head to the right, to face the crowd. In the next shot, he's suddenly looking straight forward. (00:20:45)

Audio problem: When Plutarch tell Katniss she has 10 minutes to show her skills, the audio doesn't match his mouth. This is not due to him being behind a force field, as demonstrated earlier in the film. (01:04:50)

Continuity mistake: When Haymitch is talking to the head peacekeeper, the cut on the peacekeeper's forehead disappears in some shots and reappears in others. (00:36:10)

Continuity mistake: When the spool of wire snags, the cable is running from the left hand (screen right) side of the spool. Camera changes and now the wire is running from the right hand (screen left) side of the spool. Despite the fact the wire is stuck and won't move. (02:00:35)

Continuity mistake: When Katniss and co get attacked by the monkeys, she fires all her arrows or loses them in the water. We then see her run to the beach with the others. Still her quiver is empty. However, when Peeta gives her the pearl, she now has arrows. (01:42:00)

Question: During Katniss' individual assessment, was Peeta the one who painted that picture of Rue? I read that it was but that doesn't explain why Katniss got angry when Plutarch tells her she has 10 minutes to present her chosen skills. Or why she made that dummy about Seneca Crane?

Answer: It was done by Peeta, who was artistically inclined. Peeta knew that seeing it would have an emotional effect on Katniss, making her angry over Rue's death, thus helping her not to be intimidated by the Capitol judges. It was also Peeta's defiant act against the Capitol's cruelty in taking innocent lives. Katniss made the dummy of Seneca Crane as her own defiant statement. Seneca, by stopping Katniss and Peeta from eating the poisoned berries and declaring them both victors in the previous games, saved their lives, but he was later executed for circumventing Snow's orders. Katniss is acknowledging that Seneca saved her and Peeta, losing his life in the process.
