One of the times they scan back to Ben, you can see the camera man in between a girl sitting on top of the bar next to Ben and a girl sitting on a stool. This happens in between those 2 girls who are sitting to the left of Ben. It goes by really fast so you might have to pause it. (00:37:10)

Visible crew/equipment: When we first see the "Tooth Fairy" he is lifting weights in a room full of mirrors. If you look close, you can see one or more crew members reflected in some of the mirrors. The most obvious is a man standing, whom I am guessing to be the director or a grip.
Visible crew/equipment: Crew members are briefly reflected in the front of a train as we see the main characters walk away from the train. Specifically, look for a man in a red tee-shirt.
Visible crew/equipment: Very near the end, when they are doing a pan-out of the deserted building, there are three of the five characters standing in plain view. The other two are in the building, one is helping the other cope with a wound. When they do the pan-out, look through the bottom right "window hole."
