Visible crew/equipment: During the scene where Daryl's group finds the auto shop, Daryl spits on his hand to wipe the window and is surprised by a zombie banging up against the window. He says, "Got some friends inside" and walks away. The camera pans to the window and a crew member wearing a white hat and (what appears to be) an orange shirt is visible in the reflection. He seems to be taking a picture (probably for continuity, which is pretty ironic).

Continuity mistake: At the start of the episode when Tyreese and Carol are walking down the railroad tracks towards Terminus, they encounter a herd of walkers coming out of the woods. One of the first walkers to cross the tracks is a male walker wearing a burgundy sweatshirt with a rip in the middle. When gunfire erupts at Terminus, the walkers turn their attention towards it. In the next shot when they start to head towards the sound, we see the same male walker now in the middle of the herd yet to cross the tracks, next shot after that, we again see him at the front of the pack.

Continuity mistake: When Glenn wakes up in the prison you can see two chairs leaning against the walls behind him when he stands up. These are still present in the second shot, but when the camera moves back for the third shot they have mysteriously disappeared.

Continuity mistake: When Mica and Lizzie are running from the walkers on the railroad tracks, they easily outrun them and gain several meters' distance in a few seconds. When they are running to the house, the walkers have suddenly caught up and are directly behind them.
Other mistake: When Nicholas shoots himself, both him and Glenn fall with their feet facing the dumpster, and their heads facing the walkers. However, when they cut to the shot of presumably Nicholas being eaten, we see Glenn's head facing the dumpster. This does not make any sense. Glenn would have had to do a complete 180 turn before landing, or move around without being detected by walkers. In fact, the way they landed does not make complete sense at all. Another possible explanation could be that there are two dumpsters, but none is seen. The fence seen when Glenn and Nicholas climb on top has also disappeared.
Factual error: When Rick has given Andrea his revolver to end her life, he leaves the room with the others to leave Andrea and Michonne alone. The next thing you see is Rick, Tyrese and Daryl waiting outside the room. You hear the gun go off and then you hear a tinkle as a shell casing hits the floor (noticeably "lighter" than the sound the revolver would make when dropped). Revolvers do not eject a shell casing.

Continuity mistake: After the part where Merle and Daryl save the people on the bridge, Merle gets agitated at Daryl and grabs him and rips his shirt. On the left side of Daryl's back, he has tattoos on his right side, but as he walks away he suddenly has the tattoos on his left side.
Continuity mistake: Glenn demands that Abraham stop the truck so he and Tara can get off, the two male characters then have a scuffle. We can see Eugene in the background standing next to the truck facing right of the camera. When the camera cuts back to Eugene by himself, he's now facing the opposite direction. Also the items in the back of the truck move from left to right. If you look carefully, you can see that the entire image was flipped horizontally.

Continuity mistake: In the scene where Merle is the in big box late 80's chevy with big 20-22 inch rims he is shown driving it in the barn yard drinking, playing music, and zombies following him. The shot changes a few times within the same scene with the big rims on, in one shot the car is shown with him driving it but the stock rims (15") are on the car, the paint of the car looks a little darker too. Then the shot changes again while he is still driving it and the big rims are on the chevy again. The shot that shows the stock rims is a pretty fast shot so it could be easy to miss.
Continuity mistake: When the Winnebago breaks down. Shane and T-Dog offer to drive ahead to check out what could possibly be a gas station. Shane tells Rick to hold the fort while they're gone. Meanwhile Rick is dealing with dying Jim. When discussing Jim's predicament, Shane is there, he hasn't gone anywhere. Then when they put Jim under the tree T-Dog is there too. Then they all drive off like the breakdown never happened. Nothing is mentioned about whether Shane and T-Dog found a gas station and if they did, it was very quickly sorted out - two minutes tops.
Continuity mistake: As the group kills the walkers that come out of the barn, the camera shows several different vantage points from in front of the barn, inside of the barn etc., it shows the walkers lying with a clear path between them, and in another it shows the walkers lying in a jumbled pile with no path between them. This goes back and forth a few times until Sophia comes out of the barn, and there is no clear path again.
Revealing mistake: Merle is sitting in a chair and talking with Michonne and Andrea. At one point he takes off the prosthetic on his right arm to show them what used to be his hand (it's only a nub now), but shortly afterwards you can see his right hand in a fist as he puts the prosthetic back on.
Continuity mistake: On the roof all the survivors are discussing their escape plan while T-Dog is lying down injured by Merle. In the next scene people are wearing rain coats and covering their bodies with the walker's remains as Rick proposed. T-Dog has taken off his cap and looks fine. Rick gives him the handcuff keys to free Merle. In the scene after, while Rick and Glenn went out the streets, the others go back on the roof to watch them from above. Nobody is wearing a raincoat and also T-Dog is in the same condition as before, lying down with his cap on.

Continuity mistake: When Glenn is with Tara and getting ready to move out, he puts on his helmet and you can see it is strapped on. When the shot changes, he swings the fence open and you can see his strap is lose. The camera angle changes again and the strap is back on again.
Continuity mistake: In the flashback to the prison, Hershel says "we've been here two months." However, when they show Judith she is shown to be the same age she was when the prison fell (around 9-10 months) If they had only been there two months, and Lori delivered within the first week of being there, the baby should not be very old.
Plot hole: Diana in her final scene is seen shooting her last bullets at the walkers that are offscreen, moving toward her, so suicide was not an option, meaning the walkers would have eaten her until she died, becoming a walker. And yet her body is completely clean when her son and Michone discover her in walker form. Blood free (even on her clothes) and fully intact, no chunks of her missing, no bite marks.
Visible crew/equipment: Andrea and the crowd are at Woodbury main gate arguing, when they hear a woman, closely followed by a man screaming up the street. Andrea and a lookout start to run up the road, where there is a woman running down the road. A sound-man in white shorts and red shirt, holding an overhead boom mic, is visible as he starts to jog down the street, tracking the woman as she moves towards them.
Visible crew/equipment: Negan and Carl go for a ride in the big truck. Daryl tells Negan not to harm Carl and he flips him off. In this scene, the camera tracks upward and its shadow casts on the truck. Also a film light can be seen reflected in the door jam.
Factual error: Ten minutes into the episode, where they're setting up their police-blockade, a cop makes a remark about being in some TV-show when Rick Grimes replies with: "Make sure you've got a round in the chamber and your safety off". Next we see the first cop checking his gun (a Glock) for a round in the chamber and we see him flicking his thumb across the gun's slide catch: not a safety-lever. Glocks don't have an external safety, so the cop cannot flick a safety-lever like in the episode.
Continuity mistake: In Season 2 episode 7, "Pretty Much Dead Already," at the end where Rick shoots the walker Sophia, she falls to her right. In the next episode, "Nebraska," picking up immediately after the last, she is in an entirely different position, as if she fell forward.
Audio problem: When Rick and his 'strike force' hole up in the cabin surrounded by walkers. They arouse the cabin's owner who brandishes a double-barrel, breech breaking shotgun. When characters brandish pump action shotguns, often they pump a round for effect and the sound effect is characteristic. The pump action sound effect is erroneously applied to the breech breaking double barrel shotgun.
Continuity mistake: The first shot of Maggie waiting for the Governor to enter shows her wearing a black tank top with her shoulders bare. After he cuts the duct tape around her wrists, the camera pans up and she now has black bra straps showing on her shoulders.
Other mistake: In Woodbury, on the street, while Andrea talks to the Governor, we can see everybody wearing jackets and orange, red and yellow trees, showing that the scene was shot during fall. According to the beginning of the third season, they had just passed through a hard winter. So it should be spring or the beginning of summer.
Continuity mistake: When Beth is looking for a key in Officer Dawn's office, she encounters Joan lying dead in the office from bleeding out, apparently from suicide with a pair of surgical scissors. Her left hand is covered in blood. But when Officer Gorman enters the room and Joan starts to turn into a walker, the blood on her hand has disappeared.

Continuity mistake: After The Governor shows up at the prison with his arsenal, Hershel and Michonne are taken out of the vehicles and placed kneeling on the grass side by side, about 1 arm's length apart from each other. But when Rick makes his way down to the fence to talk with The Governor, Hershel and Michonne are now about 4 arms' lengths away from each other. This goes back and forth for a few shots.
Continuity mistake: When Rick, Carl, and Michonne are leaving King County (Rick's home), they must travel around a wreck involving two cars and an overturned RV. They drive along one side of the road. When the camera angle changes, they are on the opposite side of the road.
Visible crew/equipment: When Sam comes to visit Carol with items in his hand, he walks through the front door of the house. Carol shuts the door after she lets him inside. As she does this, you can see the reflection of a black sheet or tarp of some kind covering the door area where the camera reflection would be.
Continuity mistake: The previous episode ended with Rick and his group reaching Woodburry and it was dark. This episode starts with Tyreese and his group reaching the prison in the daylight, and being rescued by Carl and brought back to their cell block. If you look at the windows it is still light out, they have no electricity so it can't be exterior lighting, yet in the scene before and after this, Rick and his group are at the Woodburry at nighttime.
Continuity mistake: When helping the foreign family to fight off the walkers, Daryl pulls the zombie back into the car and slams its head in the boot door, covering his hands in blood in the process. However, for the rest of the scenes in the episode and in shots directly afterwards, his hands appear totally clean without him doing anything to clean them.
Other mistake: In Season 6, it is revealed that Maggie is pregnant. In Season 8, she's not even showing, suggesting that not much time has passed, but in that time, Judith has grown from an infant into a toddler.
Continuity mistake: In the golf club Beth is wearing her new yellow t-shirt and white cardigan. Daryl hits walker blood all over her cardigan. In the next scene she takes the cardigan off and the t-shirt underneath (which was covered by the cardigan) has blood all over it. Back in the forest after this the shirt is clean again.

Continuity mistake: Right at the end of this episode when there is another flashback to the prison, the baby is seen many times. Judith is asleep, then the camera angle changes and she is awake, it moves back to another angle and she is sleeping again. This is repeated several times.

Continuity mistake: As Daryl and Shane argue near the start, Daryl's movements don't correspond with the changing shots: one moment he is standing in front of Carol, then he is standing by her, then he has moved closer to Rick - all within the same one-liners. As his argument with Shane grows, he again is standing by Carol, or closer, near Rick. At one point, a shot shows Daryl about to lounge at Shane, as Rick blocks him, but the very next shot shows him right next to Carol, who at that point would have been standing behind Rick.

Continuity mistake: In the final scene of the episode, Rick and Hershel are talking while looking out over the prison yard. Near the end of the conversation, Rick squarely faces Hershel and says "I'm hoping you could talk me out of it." The camera then changes to Hershel, and Rick is now immediately facing 90° to his left looking over the yard. When the camera changes back to Rick, he is now looking directly at Hershel.
Other mistake: Most if not every time someone kills a walker in close proximity or repeatedly bludgeons one that is already dead with a blunt object and there is a CGI blood spatter effect (as opposed to a practical blood spatter effect), the blood magically never gets on the person's face.

Continuity mistake: Possible Spoilers! At the very end of the episode Rick walks up to the wall of Alexandria to check out a noise. When the camera shows the walker on the other side of the fence it is clear there is nothing else out there. When the camera shows the fence from behind Rick there is the roof of a house visible above the top of the fence. The shadows made by the struts of the fence make it apparent that if there was a house the other side of the fence would have been in shadow.

Visible crew/equipment: Governor is 'interrogating' Michonne privately about staying at Woodbury to "join his forces." As he mentions Merle's name, she suddenly reacts, grabbing her sword from him, and shoves it at his throat, threateningly. During this, the shadow from the boom mic is visible just above his head.
Continuity mistake: After Carol tells Shane they are not leaving without finding Sophia, Daryl enters and reminds Shane about the doll he found down at the swamp earlier, and they start to argue. During this he crosses front to Carol's right. In the very next shot Daryl is suddenly shown standing in the same spot as prior, next to Carol.
Factual error: In the opening scene and throughout, young Sam Anderson, cloistered in his bedroom, is presumably listening to a crackly 45 rpm copy of Nick Lucas crooning his 1929 hit, Tiptoe Through the Tulips. In reality, the disc with gold/blue label spinning on Sam's turntable is not the song playing. The original Tiptoe was released in 1929 on 78 rpm vinyl by Brunswick Records, and was finally pressed on vinyl 45 by Accent Records in 1974 (solid robin egg blue label however, not gold/blue).
Continuity mistake: When the Governor first attacks the prison, he is standing on the driver's side but in front of the truck. Next, Michonne aims at the truck and now the black guy who was always with Martinez is shot dead on the driver's side. Then after that, you see he's alive again on the passenger side right before the other truck comes crashing through the gates with all the walkers inside.

Continuity mistake: When Rick and Aaron are loading up the supplies from the houseboat, they put a container with a red can in straight and flush with the other container, so that the lid sticking up is to the left. In the interior shot of the van, the container is twisted sideways and pushed back with the lid to the front.
Continuity mistake: At the end 3 men go to rush Rick - the guy on the right is Glenn. All three men are standing shoulder to shoulder when Rick pulls his gun, and Glenn has disappeared. Cut back to Rick and a wide shot - no Glenn. Cut to a close up of the group and Glenn's back, but there is a big gap between him and the other two men. This scene is continuous - he hasn't got time to move.
Continuity mistake: When the Governor and his group show up and bomb one of the guard towers to get Rick and his group's attention, when Rick first runs outside the guard tower on the far left is on fire, but when Rick walks down to the fence to talk to the Governor the guard tower in front by the front gate is the one on fire. And it switches back and forth between the two.

Continuity mistake: When Carl shoots the masked man who is found to be wearing body armor, Rick pulls up the armor to find the bruised ribs of a Caucasian male. When the assailant is unmasked however we see that the Caucasian male has magically transformed into an African American man.
Continuity mistake: In the scene in the garage near the beginning of the episode where the characters are gathering supplies, the sun is on them in one shot. In the reverse shot, they are dark and the sun is no longer shining into the garage. Then back to them, it is again.
Continuity mistake: When Glenn wakes up we see the door behind him is flush with the concrete, but when he goes inside you see stairs he had to descend to get to the door. There is also no scene in the entire prison episodes where an outer orange door is preceded by steps, they are all flush. Also If you look at the door behind him you see two folding chairs leaning up against the door. When he turns to open the door the chairs are gone.
Other mistake: Straight after Carol and Mika find some houses and are leading Tyreese, Lizzie and (supposedly) Judith to them, Judith is missing throughout the entire scene. No one appears to be holding her or even bothered by the fact that she isn't there. Then she's back again in the next scene.
Continuity mistake: In the previous episode, Rick and Carol are shown hunting through houses for household drugs to fight the flu. They even make a mention that most of what they find expired a year ago, but to bring it back to Hershel anyway, and let him decide about it. But in this episode, Rick returns, talks to Hershel, and no mention is made about him bringing medicine. Considering how important it was to fight the flu with any means possible, Rick would not have let Carol drive off with the potentially life-saving drugs.
Continuity mistake: Deleted footage/special release only: Carol confronts Daryl about the boy being held hostage inside the barn (following Sophia's death), reasoning with him about pretending not to care when he truly does. At one point he moves to her, almost in her face. However, the following shot shows only a close up of Carol, implying they are standing feet apart, which is not the case.
Other mistake: As Daryl carries a saddle toward the barn, in the far shot, there is no one around. The shot then changes to him entering the barn carrying the saddle inside. Suddenly we now see Carol coming around the corner, several yards behind him. Where did she come from? Unless she was already inside next door and saw him enter, she shouldn't have been there.
Factual error: When Lilly comes across the Governor's body, she points her 1911 at him. In one shot, we see her from behind with the hammer forward on her 1911. In the next shot she immediately kills the Governor by shooting him. A 1911 will not fire unless the hammer is back.
