Arrow on the Doorpost - S3-E13
Continuity mistake: In the final scene of the episode, Rick and Hershel are talking while looking out over the prison yard. Near the end of the conversation, Rick squarely faces Hershel and says "I'm hoping you could talk me out of it." The camera then changes to Hershel, and Rick is now immediately facing 90° to his left looking over the yard. When the camera changes back to Rick, he is now looking directly at Hershel.
Other mistake: Most if not every time someone kills a walker in close proximity or repeatedly bludgeons one that is already dead with a blunt object and there is a CGI blood spatter effect (as opposed to a practical blood spatter effect), the blood magically never gets on the person's face.
Beside the Dying Fire - S2-E13
Other mistake: While the farm is being overrun, Hershel is out front with his shotgun constantly firing. He never reloads and would never had the chance to stop and reload that weapon with so many zombies on him. His shotgun appears to have infinite ammo at this point.
Continuity mistake: Possible Spoilers! At the very end of the episode Rick walks up to the wall of Alexandria to check out a noise. When the camera shows the walker on The Other Side of the fence it is clear there is nothing else out there. When the camera shows the fence from behind Rick there is the roof of a house visible above the top of the fence. The shadows made by the struts of the fence make it apparent that if there was a house The Other Side of the fence would have been in shadow. (00:48:00 - 00:50:00)
Say the Word - S3-E5
Visible crew/equipment: Governor is 'interrogating' Michonne privately about staying at Woodbury to "join his forces." As he mentions Merle's name, she suddenly reacts, grabbing her sword from him, and shoves it at his throat, threateningly. During this, the shadow from the boom mic is visible just above his head. (00:13:50)
Continuity mistake: When Andrea is attempting to retrieve the pliers on the floor she picks them up between her toes with the head facing up. In the next show the pliers can be seen with the head facing down when she grabs them with her hand.
Factual error: In the opening scene and throughout, young Sam Anderson, cloistered in his bedroom, is presumably listening to a crackly 45 rpm copy of Nick Lucas crooning his 1929 hit, Tiptoe Through the Tulips. In reality, the disc with gold/blue label spinning on Sam's turntable is not the song playing. The original Tiptoe was released in 1929 on 78 rpm vinyl by Brunswick Records, and was finally pressed on vinyl 45 by Accent Records in 1974 (solid robin egg blue label however, not gold/blue).
Continuity mistake: When the Governor first attacks the prison, he is standing on the driver's side but in front of the truck. Next, Michonne aims at the truck and now the black guy who was always with Martinez is shot dead on the driver's side. Then after that, you see he's alive again on the passenger side right before the other truck comes crashing through the gates with all the walkers inside.
Suggested correction: The man you're taking about is Shumpert aka "Bowman." He's was not shot dead. He dove for cover and is still alive. In fact, he's in at least a half dozen more episodes, including in season 4.
No Sanctuary - S5-E1
Character mistake: When Gareth is questioning Rick about what was in the bag he hid in the woods, Rick replies a .44. Rick is referring to his Python, which is a .38 or .357, not a .44.
Continuity mistake: When the walkers come through the cars, most shots show Carl and Sophia to be under the same car. When Sophia is attacked by the walker, Carl is not there and Sophia is shown on her own in other shots as well. (00:19:25 - 00:20:05)
Suggested correction: They never get under the same car. When we see them first get under the cars, they are both seen getting under cars going in the same direction at the same time (they'd have to go in opposite direction to get under the same car from both sides). Then in the first close up of Sophia, we can see Carl's head in the shadow and then a strip of sunlight indicating 2 separate cars.
Too Far Gone - S4-E8
Factual error: When Lilly comes across the Governor's body, she points her 1911 at him. In one shot, we see her from behind with the hammer forward on her 1911. In the next shot she immediately kills the Governor by shooting him. A 1911 will not fire unless the hammer is back. (00:49:00 - 00:50:00)
Continuity mistake: When Shane tackles Jim to the ground, Jim tells the group about how the walkers got his family. Jim isn't wearing his cap on his head, but as the shot is about to cut to the next one, Jim is wearing his cap.
Continuity mistake: Beta kills Marry/Gamma and places her next to a tree. During the scuffle with her, his mask was half ripped and another Whisper comes up to him, now recognizing his face. Beta kills the man, then the camera cuts back to Mary's body, and her head is now turned to the right and her hair has been brushed away from her eye. She also goes from being slouched over to nearly sitting up right against the tree. All this while she's just a dead body against the tree. (00:17:04)
Continuity mistake: After Beta kills Mary/Gamma, he drags her body and sets it up by a tree, dragging her by her hair. There is a jump cut as he walks away, and her eyes move to a different position, as well as the amount of hair over her face changes. (00:16:25)
When the Dead Come Knocking - S3-E7
Other mistake: When Merle interrogates Glenn, who doesn't seem to be very cooperative, he says, "Well, maybe the farmer's daughter will help me out." Merle wasn't on a farm, never met Herschel; he should have no idea that Maggie is a farmer's daughter.
Suggested correction: A "farmer's daughter" doesn't always mean someone who is literally "the daughter of a farmer." It's also an expression that refers to a specific type of woman and even to a type of character in movies and TV shows.
Revealing mistake: Carl is walking toward a walker embedded to a tree. In the background there's a female walker wearing a distressed white dress with long black hair and a bear trap caught on her leg, standing in the back waiting for her scene to walk out. (00:22:30)
Pretty Much Dead Already - S2-E7
Visible crew/equipment: When Lori is cutting carrots and Shane is walking up to talk to her, look in the background behind Shane as he squats down. There is a person walking towards the barn.
Visible crew/equipment: You can see the casting directors on The Bridge behind Carol and Daryl wearing lime green vests. Then below The Bridge, you can see people driving their cars and a person walking in the parking lot. (00:26:57)
The Day Will Come When You Won't Be - S7-E1
Continuity mistake: When Negan makes Rick get the axe from the top of the RV, Rick is fighting the Walkers as he heads for the RV door after picking up the axe. He slams a walker's head into the back of the RV, blood splattering everywhere. Next scene shows Negan and Rick driving back to the group, and the back of the RV is spotless.
Continuity mistake: When Rick, Michonne, and Carl go out on a run, in the first shot, Rick starts to roll the window down so he can shoot a walker. The next shot the window is up, and he rolls it down again. (00:02:00)
