Visible crew/equipment: After Sam/Jim tells Melinda that he'll be leaving and doesn't know how long he'll be gone, as he drives away the reflection of the large reflector screen can be seen on the pickup's vent window.
Continuity mistake: While Melinda's trapped underground with the rising water flooding the utility vault, when Eli and Jim go down below Eli takes off his shoes and jacket, the camera pans up from the churning water and we briefly see Eli's bare feet. The next shot we see him taking the socks off.
The One (2) - S1-E22
Factual error: The tail section of the crashed plane is from an early model 737, based on the curvature where the forward edge of the vertical stabilizer meets the top of the fuselage. The maximum seating capacity would have been about 130 passengers, while the number of crash victims was given as approximately twice that.
Fury (a.k.a. Condemned to Repeat) - S1-E19
Continuity mistake: When Melinda is lying in bed talking to her husband her hair keeps moving from all lying on the right side of her pillow to completely under her head.
On the Wings of a Dove - S1-E8
Continuity mistake: When Joseph wants proof that Melinda sees Julian, the boy asks Melinda to describe the tattoo behind his father's right ear, and we see a close-up of the white dove tattoo that Julian got after his son's birth. Then, while Julian whispers to his son, there is no tattoo behind his left ear, but as Julian walks backwards toward Jim the white dove tattoo is now visible. (00:37:40)
The Walk-in - S2-E17
Continuity mistake: When Jim brings a new body to the morgue there's a "John Doe" already lying there, and in the close up we see that the corpse's right foot is missing the middle toe. We see this again later when Jim's ring is stolen from his locker, with the bloody right footprint on the floor. However, when Melinda shows up the close-up shots of the bloody footprints have been flipped so it's the left foot with the missing toe, but what's funny is that in the audio we hear Jim say, "The middle toe is missing on the right foot."
Other mistake: When Rick suggests that the ghost may have had multiple personality disorder and that's why he's appearing as different people, Melinda says this isn't possible, because when someone dies they're free from all afflictions they had when they were alive. In season 2 episode 4, "The Ghost Within", the featured ghost has autism and is not able to verbally communicate or make eye contact with Melinda and we never see him freed from this mental affliction.
Character mistake: When Delia is rushed out of the shop at closing time by Melinda she is wearing gold long 2" chain earrings. When she gets to her vehicle and reaches for her gift she is wearing gold filigree hoop earrings.
Imaginary Friends and Enemies - S4-E6
Continuity mistake: In the scene in the woods where we first see Owen's ghost, as the camera cuts back and forth between Melinda talking to Owen and what her friend sees, the two women are alternately closer and then significantly farther apart. They're supposed to be standing basically in the same spots, only taking maybe one or two small steps during Melinda's conversation with Owen.
The Prophet - S2-E21
Continuity mistake: Melinda calls Deliah early in the morning. Her husband just said it was 4:10 and she called her friend who also said it was 4:10. Deliah looks at the clock and confirms the time. But as the camera pulls back, the minutes on the clock are visible, showing 22.
Revealing mistake: When Melinda goes to Justin's dorm room to speak to him after he dies, the camera pans to his body on the bed and you can see that it's a body double. (00:24:50)
Continuity mistake: When Eli tries to use a card to open Zoe's door, the card breaks. He is holding on to the larger piece of the card in his right hand when this happens and the smaller piece is in the frame. He gets the other piece and the shot changes to him suddenly holding the smaller piece in his left hand, as well as the card having been flipped around (the strip is seen in both shots, and the two shots face in opposite directions).
Continuity mistake: The lights over Melinda's shop appear and disappear all through the entire show.
Continuity mistake: After Delia and Melinda are at the house in the countryside the episode cuts to a scene at Same As It Never Was Antiques. In this shot out the front we can see Melinda's 2005 Jeep Liberty Limited edition parked, but Melinda hasn't had this since the first episode of season 3. From Season 3 episode 2 Melinda can be seen driving a 2007 Saturn Outlook XR. (00:04:50 - 00:39:51)
Continuity mistake: At the beginning of the episode when Jim and Melinda are watching the last of the movie, at one point Melinda takes a drink of her beer and crosses her arms twice but from a different angle.
Other mistake: The show was supposed to be on the East Coast, however you can see a reflection of a California Flag in Melinda's front windshield. (00:09:35)
Character mistake: Jim tells Melinda a story of how he and his father watched a baseball game in which Reggie Jackson hit four home runs in one game. The largest amount of home runs that Reggie Jackson ever hit in one game was in the final game of the 1977 World Series, in which he hit a total of three home runs.
Character mistake: Jim says that the last man executed in the USA by hanging was Billy Bailey on September 25, 1996. This is not correct. It was actually on January 25, 1996.
Continuity mistake: When Melinda is walking to Andrea's apartment and is stopped by a news crew for an interview, she gives her interview and says "We're going to be okay" at the end of it. In the scene in Seattle where someone that knows her sees her interview on TV, her interview is different in that her manner is completely different and happier and she says "we'll be okay" at the end instead.
The Walk-in - S2-E17
Revealing mistake: When Jim and the mortician are about to transfer the dead guy from one table to the other, his arms move slightly.
Answer: This "spirit" appears in several episodes throughout the rest of the season including the season finale. He's a "bad spirit" that is intent on keeping souls trapped on Earth. With the death of Andrea (her business partner) he wanted to make a deal with Melinda, to give him HER soul to release everyone else including Andrea to cross over.
