Continuity mistake: The red-marked leader of the ARC Troopers sure goes through a rapid succession of field promotions and degradations. In Chapter III, he is addressed by Obi-Wan as a Sergeant; when Obi-Wan arrives at the Banking Clan's HQ in Chapter IX, he calls him Commander; and when Anakin returns from his duel with Asajj Ventress in Chapter XX, he is suddenly a Captain.
Continuity mistake: After Padme drags Threepio away as the attack on the city begins, Mace Windu's fighter swoops across the screen from left to right, and it rolls onto its side to show us the top of the cockpit. We see his canopy is similar to the design found in the 'Revenge of the Sith' film, being comprised of three large panels of glass. Unlike the film version (which has a small porthole behind the pilot's head), there is no glass behind Windu. But when the shot cuts to show him inside, there are three windows behind him. Seconds later, another interior view shows the three 'windows' are now solid panels.
Continuity mistake: When Count Dooku and the disguised Asajj Ventress watch the fighters in the Rattatak arena, the lodge they sit in at first seems almost level with the arena floor. Later, when Dooku floats to the arena floor after Asajj has finished all enemies, the lodge is high above the ground.
Continuity mistake: As the ARC troopers' gunship breaks off from the rest of the ships and as it's flying through the city, its markings are blue. But as the ship is hit and crashing, its markings are the red. (It is definitely the same ship, as we see the ARC troopers emerge from it after the crash.).
Continuity mistake: When the Sniper Droid zeroes in on an ARC Troopers head, we see them in a ring around the gunship, with 2 ARC Troopers on top of the wreckage, but in the shot after that we see them in a completely different position, with all the ARCs in front of the ship.

Other mistake: In the Banking Clan's HQ, San Hill assesses the situation: "Those guns are destroying our forward positions. The Republic is breaking through!"; then the ground shakes when the giant cannon outside is blasted, and Hill flinches in surprise. But if you look closely, he flinches just before the explosion and the resulting tremor come in.
Continuity mistake: When Durge fires his in-built flamethrower at Obi-Wan, the Jedi Force-crushes the weapon. but in the scene where Obi-Wan's hand is closing into a fist, the flame stream stops just before that; and when the scene switches to Durge's arm, the flamethrower is still firing just before its muzzle is crumpled up.
Deliberate mistake: At the beginning of the chapter, as the scene switches to the surface fighting on Muunilinst, we see a score of droids blasted before the Republic Army's heavy artillery is brought in. But there are no remains on the droids to be seen after the smoke has cleared, whereas there should be something left.
Continuity mistake: During the first duel with Durge and Obi-Wan, Durge's bike is totalled and Obi-Wan's gets hacked to pieces as Durge swings it at Obi. However, as soon as the fight is over, Obi-Wan picks up his bike, completely undamaged, and goes off on his merry way. None of the other trooper's bikes are ever seen anywhere near them.
Continuity mistake: When Palpatine and his rescuers are in the descending elevator, a wide shot shows the trooper with the rocket launcher standing at the far left of the shot, in front of the hammer-headed Jedi. The shot then tightens in and suddenly this trooper is behind Palpatine, who is behind that Jedi.
Continuity mistake: When Palpatine and the others exit the elevator, a rear shot of their group shows that one trooper holds his rifle, but the other is missing his rocket launcher. But when seen from the front again, this trooper has the very obvious launcher hanging from his shoulder, behind him.
Continuity mistake: When Palpatine and his rescuers emerge from the elevator, the characters' positions in relation to each other changes radically despite nobody moving. First shot, the line up, left to right, is Hammer Head, Trooper, female Jedi, Palpatine, Trooper and four-eyes (the 3 Jedi are in the front row, the rest behind). In the next shot, the order is now (right to left, from behind) Hammer Head, female Jedi, Trooper, Palpatine, Trooper and four-eyes (Palpatine now in the front row). Next, a tighter shot has Hammer Head moved to the back row, behind a trooper. Meanwhile, the two troopers keep switching sides, as the rifle is seen at left or at right, while the rocket launcher is not seen at all until the troopers are suddenly together at the left of the screen when Hammer Head yells.
Continuity mistake: When Shaak Ti, Ronet Corobb, and Foul Moudama try to get Palpatine to safety, there are two Clone Troopers with them. As they stand waiting for the elevator, both Troopers are armed with blaster rifles, but as they walk into the elevator, one of them suddenly has a rocket launcher instead of a rifle.
Continuity mistake: After Anakin and the most recently mutated Nelvaanian free the other mutated, the latter puts his hand on one of his fellow's shoulder. When he does, the control harness the two should have destroyed earlier is still on; in the next scene right after, the harness is gone.
Other mistake: When the most recently mutated Nelvaanian tries to show Anakin how to free his tribe mates, the second Nelvaanian he rips the control harness from, suddenly has his implanted left arm blaster missing; instead his hand - which should have been amputated - appears.
Continuity mistake: When Durge attacks Obi-Wan and the ARC Troopers in the Banking Clan's HQ, San Hill cowers beneath the holo-map table. Durge jumps on it, breaking it cleanly in two, and Hill is nowhere to be seen; and after the fight, when the Troopers lift the table fragments apart, Hill is seen cowering and whimpering right behind it.
Factual error: After Oro Dassyne has boasted about his fortress' defenses and the droid standung by him announces that he has spotted two Jedi approaching, Dassyne snatches the binoculars from him to take a look himself. Curiously, he holds them at a very different angle, i.e. he is not looking into the same direction the droid was looking, and still he spots the Jedi immediately.
Continuity mistake: When Mace Windu intercepts Grievous at the latter's gunship, Grievous activates his lightsabers while standing on the ramp to his ship's hatch. But when Windu crushes his chest and Grievous starts to cough, he is suddenly standing right inside the hatch.
Deliberate mistake: When Mace Windu jumps aboard the gunship and orders the pilot to turn around, and the pilot replies with "Yes, sir", there is a blaster cannon mounted in front of the pilot canopy. However, LAAT gunships do not have a blaster mounted right before the canopy. This shot evidently comes from the scene in Chapter XXIII where Windu rescues an ARC-170 fighter from three vulture Droids.
Continuity mistake: When Anakin sucks the worm into his mouth, the sewer map is revealed underneath. But there is also a heap of dirt and bugs on the parchment when he sucks the worm in; while in the shot where the map is seen in full, there are only a few bugs scattered on the sheet.
Continuity mistake: When Anakin and Padme kiss in the alleyway, an alien passes by, glancing at them, and they break up. But in the first shot (from the main street side), when the alien looks at them it is almost past the alleyway. But in the next shot, when Anakin and Padme break apart, the same alien starts walking by the opening once again.
Continuity mistake: When Mace Windu cuts his way into the Seismic Tank, a score of droids is waiting for him, including one to the left of the opening. He still stands there as Windu completes the cutting, but the patch Force-smashes only into the droids right in front of the opening; the lone droid should be unaffected due to his position. But when Windu jumps in, the droid is suddenly gone.
Other mistake: When the Clone Troopers dispatched to protect Anakin arrive on Yavin 4, Anakin addresses their leader as Sergeant. As stated in several sources, Clone Sergeants have green markings on their helmet and arm armor, but this one has the plain white armor of a regular Trooper.
Audio problem: When the Jedi Council are discussing Anakin's promotion to Jedi Knight, in a wide shot, we hear the voice of the female, dark-skinned Jedi saying, "Yes. We need more knights." But her mouth is closed as she says this, and the male Jedi Even Piell (the Jedi talking in the previous shot) is moving his mouth as if he's the one talking.
