Continuity mistake: When the TV star is killed he is wearing a white t-shirt with black lettering. As he is dragged to and placed in the trunk of the car it is still visible. When the CSIs move him from the trunk onto the gurney, there is no lettering on the t-shirt.
Continuity mistake: When Eric is at the rehab center where the TV star was being treated, the camera zooms in from above. Seconds after his chat with the girl to whom the star had confessed murder, the camera zooms back out, but this time, the juices are rearranged, there are new white flowers by the tree on the right, the table is pulled back from the chair, the chairs are no longer right close together, there is a suspended piece of equipment above the table, and there is some funky doodad leaning up against the table that wasn't there before. The sun moved by at least a couple hours during their 30 second chat.

Continuity mistake: When Horatio leaves Dr. Bradstone's cell, after informing him that his wife was having an affair, Bradstone's arms change position back and forth from across his stomach to behind his head. (00:34:25)

Continuity mistake: When Horatio notices the Band-Aid on Nathan Cole's finger, and Dr. Ray asks if they can get a warrant, Horatio's position changes between shots from facing Ray directly then has rotated position about 90° and is looking at Ray over his shoulder. (00:29:35)
Continuity mistake: When Eric and Natalia determine that their murder weapon is the top from one of the limo's alcohol bottles, the angle of the top in Delko's hands changes between shots, without him actually moving. (00:07:30)
Rock and a Hard Place - S6-E19
Continuity mistake: When Alexx talks to Delko in the lab about her son, Delko's hands are resting on the fuming machine in front of him, then instantly on the table top. (00:29:05)
Answer: Earlier in the episode, Horatio tells Mac that Penrod was seen in Tribeca getting his leg set, only to bolt when he learned a nurse wised up the cops. Caine and the NYPD probably got Penrod's address from the hospital.
