Question: Can anyone explain the title of this episode? I understand the wordplay on "sacrificial lamb", but why "clam"?

Continuity mistake: The woman in the wheelchair that Dr. Cox claims is dead is wearing glasses in some shots but not in others.

Trivia: Ben Sullivan, Dr. Cox's best friend and brother-in-law, loves his camera and takes pictures everywhere he goes. Brendan Fraser, who plays Ben, has a great interest for photography in real life.

Answer: The title could be interepreted in relation to the patient who JD is afraid to treat and how the room is closed off like a clam. In the episode the patient has to be quarantined to a seperate room from everyone else and medical staff that enter the room have to take precautions from entering.
