Continuity mistake: When Dan's body is put in the grave, his shirt is only slightly raised up his chest. When they start filling the grave in, his shirt is raised a lot higher. (00:37:00)
Audio problem: Alan gives Teri the address near the end and says they should tell the police. Teri responds that "Jack is the police" however this line is a clear overdub as the audio is completely different to the rest of the scene. (00:36:50)
Continuity mistake: When Mrs Palmer is walking towards the senator, she has her left arm outstretched in the front shot and not in the rear shot. (00:15:00)

Audio problem: When we see the timer for the bomb on the plane, you hear the beep to indicate each second it clicks down. However, we see the timer say 24 seconds. We hear several beeps and 7 seconds later the timer says 20 seconds. The key point of the program being real time, so the timer should be on 17 at least. (00:37:30)

Continuity mistake: After Jack kills the Chinese agent, he picks up the agent's weapon (a HK G36). The weapon has a magazine in one of the camera angles but when the scene switches to another angle, there is no magazine in the rifle. (00:32:15)
11:00 P.M.-12:00 Midnight - S1-E24
Other mistake: Mason asks Nina the ETA on the tactical teams, to which she responds "Five on the Alpha, Eight on the Beta." There's a timecheck 3 minutes later, and after the break they still haven't arrived. Team Alpha continues to be more and more late than estimated. Jack begins to break into a van, still no tactical team has arrived. Jack kills Victor Drazen at the 17:47 time check, still no back up. They finally show up at around the 31 minute mark. Day 3: 4:00 A.M.-5:00 A.M. - S3-E16
Continuity mistake: When Michelle shoots the escaping hotel customer, her gun shows a jammed shell casing while she points her gun at him. The jammed casing wasn't there after she fired her final warning shots and disapears suddenly just before she shoots him down. (00:39:00)
Day 5: 5:00 PM - 6:00 PM - S5-E11
Continuity mistake: When Agent Aaron Pierce steps out of the helicopter, the right half of his face is severely wounded, with lots of blood and a bandage on it. 12 minutes later, when he is back in the Presidential house, he's talking with Miss. Logan and his face is completely clean, with not a single scratch or sign of wounds on his right half of the face. (00:12:55 - 00:25:00)
Revealing mistake: Tony Almeida tries to phone with a telephone, which is not switched on. You see that the screen isn't displaying any information. (00:25:10)
Day 8: 4:00 P.M. - 5:00 P.M. - S8-E1
Revealing mistake: When Farhad Hassan brings the press ID for Meredith Reed to security, you see the guard walk through the metal detector wearing his police belt, but the metal detector does not go off, showing that it's either not real, or not turned on.
Day 4: 10:00 P.M.-11:00 P.M. - S4-E16
Continuity mistake: When Jack shoots Nicole through the wall, she slides down, smearing blood on the wall. However when Jack comes to check if she is dead, no blood is seen on the wall.
Day 4: 12:00 P.M.-1:00 P.M. - S4-E6
Revealing mistake: During the rescue, a few shots after SecDef Heller announces to Jack that he's also out of ammo, you see a shot of the bullet-riddled car taking a few more bullets. The new bullet holes obviously fade in.
Chosen answer: Nobody knew, not even the writers, until at least halfway through the season when they were writing the end. They wanted to keep all options open.
