Visible crew/equipment: After Tara is found in the Janitor's closet and brought back to her room as Willow & Buffy come in, there is a mirror to the left of the door. It has a scarf draped over it to prevent it from reflecting things it shouldn't, but the bottom right corner is still exposed and in it you can see glimpses of the crew moving around behind the camera.
Visible crew/equipment: When Buffy, Willow, Xander and Giles all agree that Morgan is their one suspect, a camera and a cameraman can be seen in the reflection of the glass in the door labeled "Librarian", right after Buffy says, "Uh, priority check, Giles." A boom mic is also reflected in the top right of the glass when Buffy finishes her line with "talent show murder!" The mic can be seen throughout the rest of the scene from two different camera angles.
Visible crew/equipment: While the house is becoming "Wild", Riley takes Graham down to The Initiative. Once the door closes behind them and the mirror returns to its original position, the shadow of a camera is reflected in the mirror. It's in the middle of the frame, on the left, over a book case.
Visible crew/equipment: After the history teacher writes, "Don't walk away from me, bitch!" on the chalk board, Buffy says to Xander, "I'm telling you, something weird is going on." As she says "something", a boom mic is reflected in the window behind her on the right.
Visible crew/equipment: When the Scoobies are attempting to leave Buffy's house by using magic, we see a close up shot of the house's front door knob. None of the Scoobies are reflected in the door knob even though they're standing right in front of it. There is also a fairly large black object reflected in it. A film crew perhaps?
Visible crew/equipment: When Dawn is possessed by the demon and breathes fire in the magic shop, in the bottom right corner of the screen in one of the shots, a metal tube can be seen blowing fire onto the set when it should be coming from Dawn's mouth. A hand can also be seen holding a blow torch, igniting the tube and then quickly pulling out of shot.
Visible crew/equipment: During the scene where Willow and Anya fall in love with RJ after he visits Buffy's house, and everyone but Dawn runs off to do their thing, the camera pans to Dawn and a woman with a maroon, long sleeved shirt is briefly visible standing on the stairs to Dawn's right.
Visible crew/equipment: When Buffy is searching through the backstage area (before the chandelier falls on her), there is a mirror in the background. At one point in the mirror, you can see the reflection of a crew member. Pause for a better look but it can be seen at normal speed.
Visible crew/equipment: When Dawn and the Potentials try to get past one of the police officers at The Bronze, Kennedy says that he'll have to shoot them all to stop them. As he responds with "It doesn't really bother me", the shadow of a boom mic can be seen on the wall behind him in the top right hand corner of the screen.
Visible crew/equipment: After Anya has rescued Xander from drowning, they venture through the vines to the room that Buffy and Riley are in. The moment before Anya screams, "Xander, help me," a vine attacks her, but a crew member's hand can be seen (in the wide-screen version) moving the vine towards Anya.
Visible crew/equipment: In the last scene after Buffy jumps through the window into her mother's room, Faith and Buffy begin to fight. The two wrestle into the hall and begin to roll down the stairs. There is a side view shot then a front view shot. In the front view shot you can see a camera moving downward just outside the bannister and out of shot. Only visible in widescreen versions.
Visible crew/equipment: When Buffy and Xander are doing some research, check the left side of the screen in a wide shot, when Xander asks "how far back do the disappearances go?" You may need to turn the brightness up, but there's a boom mike bobbing around.
Visible crew/equipment: When Xander is leaning over the hole in the cemetery into which Buffy and Dawn have fallen, he causes the edge of the hole to collapse. Seen from below, everything is fine, but the shot from above Xander shows a hand grabbing hold of the back of his trousers and his briefs are visible.

Visible crew/equipment: Just after Dawn says "I like the trailers", we fade to black/commercials, then up again. Keep a close eye on the left side of the screen - you can see what looks like someone's hand flash into frame for a second, despite Dawn being alone in the room - looks like the director's hand signalling action.
Visible crew/equipment: When Xander is in the bathroom after Buffy called him to look for the talisman, he bends down and grabs the talisman that lays under the sinks. As he stands the camera pans up to the mirrors and on the left side you can see the reflection of a crew member crouching over the hole to the old school. It's not the female manifest spirit as the clothes are wrong and she then appears from a different part of the room.

Visible crew/equipment: When Willow and Dawn are running away from the demon, Willow uses magic to unlock a car for them to escape in. On the right side of the screen, shadows of crew members can be see as the car's doors open and are gone when Willow uses magic to make the car drive away.
Visible crew/equipment: When Buffy and Dawn are at the top of the metal tower, and Buffy is still very dazed from being called back, there is an overhead shot with a piece of the tower falling down; if you look left of the falling piece, you can see a guy wearing a hard hat walking about.
Visible crew/equipment: When Xander and Anya pull over in the ice-cream van, the front windscreen is very reflective. To avoid reflecting the camera crew, a giant screen was put in front of the ice-cream van - which, ironically, is reflected in the windscreen. It isn't a reflection of another vehicle parked in front of them as the thing that is reflected is a curved shape.
Visible crew/equipment: During the scene of the siege at Giles', there is a shot where an Indian guy jumps through the window and helps himself by holding on to the sill. If you look to the left side of the screen, you can see it's a moving wall and that several hands of crew members are holding it so it doesn't shake.
Visible crew/equipment: In the scene where Willow visits Xander to discuss what may be going on with Oz; towards the end of the scene, when the camera is on Willow, you can see the sound mike at the top left of the screen next to the green crate in two different shots.
Visible crew/equipment: There's a boom mic visible when Cordelia is talking to Harmony and the other Cordettes. It's the first scene in the alternate universe and they're standing inside a classroom of Sunnydale High (Cordy says something about going to the Bronze).
Visible crew/equipment: After Anya teleports to the jail leaving Buffy and Xander in the forest, the shadow of cameraman walking backwards can be seen as Buffy and Xander make their way to Xander's car. Look closely, the shadow is visible and Buffy tells Xander not to be too sure about Anya being able to handle Willow tonight.
Visible crew/equipment: Near the end after freeing the prisoners, Buffy fends off Ken's henchmen on a raised platform. At some point during the fight she is grabbed by one of the henchmen and punched in the stomach by two others. She then breaks free and throws one of them off the raised platform - onto a large crash mat. Please note that this is a separate mistake to the one already submitted as it is talking about a different crash mat.
Visible crew/equipment: Buffy and the gang are in the waiting room after being told the cause of Joyce's death. After Willow asks "Why don't we sit down?" the camera pans from right to left and the camera focused on Dawn becomes visible at screen left. Some frames later it appears again as Dawn leaves to "pee" and the camera zooms out.
Visible crew/equipment: When Buffy and Riley are talking at the crime scene, where he tells her about Maggie Walsh's death, one of the camera shots has a very visible black border around the entire edge of the screen as if the lens shade was visible in front of the lens and could be seen in the shot.
00:18:20 - 00:19:40
Visible crew/equipment: In the scene where Spike escapes from the initiative a black curtain can be seen to the right of the screen after the other vampire is release from his cell. The curtain serves no purpose to the set or scene and it disappears in the next shot.
Visible crew/equipment: While Buffy, Giles, and the gang are in the library fighting the demon that came out of the Hellmouth, the camera zooms toward the door that leads into the hall, where you can see Xander being chased by the dead guys. As the camera zooms in closer towards the round window in the door, you can see its reflection in the glass when the strobe light flashes.
