Revealing mistake: After Squad 51 is dispatched to the 'electrocution in a house', while Johnny goes to the garage to turn off the power, Roy waits at the doorway to go inside to the victim. In this interior shot facing the doorway, we see what is supposed to be the brick chimney at the side of the house next door, and the top of that set is visible. Plus the fact that this chimney is not nearly as deep as the one they passed by in the exterior shot.

Revealing mistake: When the Bengal tiger is on the roof, just after the officer warns, "If that cat tries to leave this roof, he's dead before he hits the street," there is a closeup of the tiger and the empty landing below the billboard. A few shots later, after Les steps on the glass, there are pieces of meat scattered on the landing below the billboard, so the trained tiger will jump onto it.

Revealing mistake: At the start of the episode, we see Squad 51 speeding toward their destination, and when the camera cuts to the woman in front of the market, she's waving her arms to the squad somewhere in the distance. But notice that while Rosa is looking to her left and waving, we can see Squad 51's reflection in the window (with the liquor bottles), as they are parked right there, just off-screen waiting for their cue to "speedily" pull up.

Revealing mistake: When Johnny and Roy are figuring out how to free the woman trapped in her wrecked car, the flames spread and Johnny's pant leg catches fire, but as he's trying to smother that flame, the strip of fire stunt gel that was smeared on his pant leg is visible.

Revealing mistake: When Captain Stanley runs into the burning house looking for Johnny, the floor beside the stairs (under the table) is visible behind him, and there is no rug only wall-to-wall carpet, but when Johnny and Toby fall from the stairs, that area of floor is covered in a rug raised up like a platform, covering the crash mat for the stunt, then the rug vanishes when the guys get Johnny and Toby.

Revealing mistake: When Squad 51 is dispatched to the man who has difficulty breathing, once they arrive and it cuts to a closeup, at the top of the screen there is a small dark film square that moves towards the left corner, and then it stays there in the following shots facing Johnny, until he contacts Rampart on the Biophone.

Revealing mistake: When Johnny, Roy, Vince, and the wounded officer are hiding between the bakery truck and car, before the multiple bullet holes actually appear on the side of the truck, those areas where the holes will show up have off-color markings, prepared for the stunt.

Nurse's Wild - S1-E7
Revealing mistake: When Gage and DeSoto are on scene with the unconscious woman and a dog, they break the front window of the house. In the interior shot as DeSoto climbs through the window, there is a white wall beside the window outside, which blocks the view of the "outside." That wall does not exist in the exterior shots. The same thing happens when Gage opens the front door.

Revealing mistake: When Cindy walks in mid-air Matrix-style, she knocks the killer backwards. In the next close-up, as he falls on his back, the outline of the thick protective padding worn under his clothes is visible.

Revealing mistake: When the killer throws Cindy against the wall, causing the painting to fall, the outlines of the knee pads she's wearing under her pants are visible, while she's in mid-air. (01:14:20)

Revealing mistake: After a naked Chaucer tells William of his gambling problem, and Simon the Summoner says that he owes 10 gold florins, when Wat angrily grabs Chaucer to punch him we see that Chaucer is actually wearing some type of underwear in the close-up. (00:26:05)

Revealing mistake: After Axel gets off the phone with Jan, when the trio are driving to the racetrack, in one shot Axel has his arm out the passenger side window as he's holding the red police beacon on the car's roof. Note the stunt double's gold watch instead of Axel's black watch. (01:19:45)
Revealing mistake: When Elizabeth first sees the skeletal pirates she stumbles backward, and when she lands on the tarp in the close-up, the flesh-covered thumb and part of a hand is visible behind her, at the top right, though the pirates are in skeletal form. (00:59:05)

Revealing mistake: At the James S. McDonnell Space Hangar, upon finding Jetfire, Wheelie turns and says, "This guy's a legend," and then later Wheelie turns and says, "I tell you, this guy did not age well." These specific CGI frames of Wheelie are the very same frames, and only the background plates (with and without Jetfire) differ. (01:22:40 - 01:23:40)

Revealing mistake: When Sam, Mikaela, Simmons and Leo are being pursued by the police in Egypt, one of the police cars slams into a parked red car. This shot has been flipped. Note that the driver is on the right side, whereas in other shots the drivers are on the left. (01:32:44)

Revealing mistake: When Mikaela is driving the Saturn she's wearing a long sleeved leather jacket, yet when Grindor shows up in front of them and the car spins, the driver is wearing a short sleeved t-shirt. (00:54:00)

Revealing mistake: When Hooper is in the cage the regulator hose is to Hooper's right (viewer's left when facing him). However, when the shark first appears and passes Hooper, in the shot facing him the hose is now to Hooper's left (viewer's right), and the word "repeater" on the poison shark dart is also backwards. Revealing mistake: At the Smithsonian Air and Space Museum, there is a triangular enclosure around Jetfire that amusingly just fades away and vanishes as Jetfire transforms. (01:23:05)

Revealing mistake: When Sam is standing on the stairs with his hands up, just before Megatron grabs him and throws him onto the table for the 'Doctor', the rail and balusters in front of Sam have been set up to easily break for the stunt - note the short diagonal bits of wood propped up between them, precisely where they break for the grab. Revealing mistake: When Grindor drops the Saturn with Mikaela, Sam and Leo inside, in the side shot as the airbags deploy, the stiff, shiny mannequin dressed as Sam is much too obvious through the open window. (00:55:40)
