Quantum Leap (1989)

Show generally

Revealing mistake: Almost every time Sam looks in a mirror or the viewer sees Sam in a mirror, the movements of Sam and Sam's "reflection" hardly ever match up correctly as if it were Sam's real reflection.

Continuity mistake: At the beginning of the film when the Russian troops are rushing headlong at the Germans, there is an obvious cut in one shot as several people shift places and one Russian just disappears.

So Help Me God - July 29, 1957 - S2-E9

Factual error: At one point when Sam is visiting Delilah in the courthouse he is hugging her and the Sheriff guarding the door comes in and sees them. As the sheriff is leaving, he has a patch on his arm that says "County Sheriff." As this episode took place in Louisiana, there would only be Parish Sheriffs, not a County Sheriff.

Fahrenheit 932 - S1-E12

Deliberate mistake: When the runner is shot in the back of the head in the SUV, the bullet has come out the front of his head. However, blood is seen on the windshield instead of a bullet hole where the bullet would have continued to go through the glass.
