Visible crew/equipment: In the scene where the convoy is driving through the entrance of the tunnel you can see the black helicopter the film crew uses right behind the police helicopter. (01:15:15)
Continuity mistake: When Batman is about to glide into Lau's building they show him standing on the ledge about two meters from the corner, ready to make his jump. Cut camera angle to an overhead shot and he is standing on the corner of the building when he makes his jump.
Continuity mistake: In the car chase, as they exit onto lower 5th, the convoy has, in order, a police cruiser, 2 swat vans, then 2 cruisers. When the garbage truck pulls up next to the convoy, he takes out the second to last cruiser first (there is a car visible behind). It then spins out another cruiser in front of it before reaching the van with Dent in it, but there should have been no other cruiser between that one and the van. (01:13:00)
Continuity mistake: When the Joker is taken to Gambol, he shoots a pool ball. We hear it connect with another ball, and then five seconds later the ball reappears. When it reappears it is moving too fast to have taken the time it did. (00:28:30)
Continuity mistake: Just after Gordon gets a call from Batman saying, "Gordon, I have the Joker's location, Pruitt building" it cuts to an aerial shot of the building. On the building there is a construction banner with the logo DAVIS, with a blue anchor shape logo. Then later on, when Gordon has the binoculars and says, "Why would they choose a spot with such good weakness" there is another aerial shot where the building banner now read BOVIS, with a blue arch logo. (01:56:55 - 01:58:55)
Continuity mistake: When Joker is looking for Harvey, he grabs Rachael and holds her head. Camera cuts and his grip on her head/face changed instantly. (00:49:25)
Other mistake: During the Prewitt building siege near the end of the movie, three SWAT members are rappelling though glass windows to challenge some hostages who are dressed as clowns. Batman runs towards them and on screen just in time mid sentence to defeat two of the SWAT members with the third member already inexplicably writhing on the floor in the background, as though he has already been downed by the Caped Crusader before confronting the other two. (02:07:05)
Factual error: When Fox is talking with Lau over lunch on what is supposed to be the roof of Lau's Hong Kong office building, you can see the buildings of Chicago's McCormick Place in the background. (00:29:55)
Revealing mistake: In the scene where Batman is taking down the SWAT members to prevent them from harming the wrong people, one of the SWAT guys is lying on the ground and attempts to stand up. The clip for his gun contacts the ground and flexes like a rubber stunt gun would, not a real clip and gun.
Continuity mistake: When Harvey is talking to the fake cop, he hold a coin and says, "Heads, you keep yours, tails, not so lucky." When he says this, the way he holds the coin changes instantly. (01:04:10)
Factual error: As Two-Face, Harvey Dent is missing his left cheek and all of his left lips, yet he is able to talk without any severe speech impediment. Not possible; although people with similar afflictions are able to talk, there is a discernible difference in not only the words they speak, but how the words are formed. Speaking words with the letters "P" or "B" require either a closed mouth or pressing the tongue against the upper lip. Two-face was unable to do either, yet speaks perfectly.
Other mistake: Whenever a character shoots a shotgun in the air inside, there isn't any debris from the shot. It happens when Joker crashes the fund raising party and when guards try to control the prisoners on the ship. (00:47:00)
Factual error: When Batman jumped and landed on top of the van driven by Scarecrow, the van stopped dead in its tracks. The forward momentum of the van would have kept it moving, even if it were undrivable. The van stopped at the precise moment Batman landed on it, so Scarecrow didn't have time to slam on the brakes.
Visible crew/equipment: When the Joker gets on the bus, after blowing up the hospital, about halfway along inside the bus is a large camera set-up. It can't belong to Mike Engel's news crew as it is not only too large to be hand held or shoulder mounted (it is one of the IMAX cameras used to film a shot from within the bus that wasn't used in the final cut) but also has a thick black cloth draped over it for the cameraman to work beneath. Obviously not a camera used by any news crew. (01:48:00)
Factual error: Batman jumps from Hong Kong's IFC (International Finance Centre) Tower 2 to IFC Tower 1 (where Lau's office is located). But when you see the lobby of the building (IFC 1) where Lucius Fox left his phone, it's actually the lobby of another building in Central Hong Kong. That building is The Center. Also, I'm not sure where Nolan shot the meeting between Fox and Lau, but it was definitely not IFC 1 as we can see the Hong Kong Four Seasons hotel in the background, and that building is next to IFC 1, not across from it.
Continuity mistake: In the scene when the Joker talks to Harvey in the hospital, Dent's head changes position from the left side of the pillow to the right.
Revealing mistake: When The Joker destroys the hospital the windows on the second and third story don't reflect anything. This is because the day before shooting at the abandoned building vandals broke in and destroyed the windows, meaning they needed to be replaced by CGI. Confirmed in the "Making of". (01:52:35)
Continuity mistake: When Batman is removing the part of the wall with the bullet in it, we see the bullet hole is off-center, to the left side of the block that Batman carves out. In the next scene where we see the mounted gun shooting specialized bullets into blocks, Bruce compares the block he removed from the apartment, which is in a plastic bag, to the holes left in the sample blocks. The hole in the block from the apartment is now in the center of the block, not on the left side as it was when he removed it.
Continuity mistake: When the Joker first opens fire on the prison van, you can see that his pistol locks open as if it were empty. However, if you look closely, you can see the empty shell is stuck between the slide and the chamber. In the next shot, he keeps on firing like nothing ever happened.
Factual error: Despite the stunt looking really cool, the flipping of the tractor trailer simply would not have occurred as shown. Assuming Batman does exactly what we see regarding the cable, the truck would have just torn the cable out of the asphalt along with the lamp posts, barely being slowed by them. Ignoring this, if the cable's anchor held firm, the cable would more likely break. Failing that, the truck would simply, abruptly stop. But in no case is it capable of generating the force required to make the trailer rise up and turn with the entire rig over its own front end. Further, the tractor and trailer are not a single, rigid piece as shown. The two pieces would form a "V" while in the flip due to the trailer lagging behind the tractor as it rotates over. Once bent, the hitch would fail and the trailer would drop back on its wheels.
Answer: It's habit, and a sensible one at that. When he's in the mask, when he's being Batman, he uses the voice, even if the person he's with knows who he is. If he drops back to his normal voice with some people, it sets the precedent for using it while wearing the cowl, which means that he's more likely to slip up and use it around people who don't know, potentially revealing his true identity. If he sticks rigidly to using the voice when kitted up as Batman, regardless of situation, it minimises that possibility.
