Continuity mistake: When Hai stabs Kuang against the side of his face, blood is shown slowly poring out of the wound on Kuang's face. The camera then cuts to focus on Hai, and the blood coming out of Kuang's wound is suddenly spurting much quicker. (00:32:32)

Other mistake: When Hai is speaking with Kuang just after stopping him from killing the bald inmate, Hai asks Kuang if he kills him, what do they say to the assistant warden. There is an error involving the English subtitles regarding Kuang's next line. Kuang's line, in the subtitles, is listed across two rows, but the rows are displayed in the wrong order. The first row reads "him, we won't take it anymore." And the second row reads "Hai, lots of men have died because of", which makes no sense when it's all put together. The full line should have been displayed as "Hai, lots of men have died because of him, we won't take it anymore." But it has incorrectly been displayed as "him, we won't take it anymore. Hai, lots of men have died because of." (This depends on which format and which studio distribution you are watching, but it can definitely be seen on the Region B Blu-Ray distributed by MediumRare). Continuity mistake: Just after Hai has stopped Kuang from killing the bald inmate, Hai leans on the staircase behind him. His elbow is shown leaning on the step at first, but in the next shot, his elbow is suddenly leaning along the pole of the banister. (00:31:40)

Continuity mistake: When the bald inmate is pushed over the staircase and cornered by the other inmates, he says "Hold it! What's this about" and is pointing with his right hand finger before moving his hand towards the ground. The camera changes angles and his right hand is suddenly reaching out again. (00:30:24)

Continuity mistake: Whilst the assistant warden has Ricky's hand trapped in his hook hand, there is a shot where the assistant warden slams his right hand into his hook hand to make it more painful for Ricky. In the next shot of Ricky, both of his eyes are shown wide open. When the shot cuts, his left eye is suddenly squinting and is closed more than the right eye. (00:25:37)

Revealing mistake: When the assistant warden stabs Ricky in the hand with his metal hook hand, in the close-up shots of Ricky's hand, it is apparent that Ricky's hand has been replaced with a fake dummy hand. (00:25:08)

Revealing mistake: When the elderly inmate is telling the two prison guards he wants to go home to see his wife and kids, one of the guards kicks him, and he falls back, but the kick misses the inmate by a few inches. (00:11:50)

Continuity mistake: At the start of the scene where the two prison guards are telling the elderly inmate that his probation has been rejected, in the establishing shot, he is shown leaning his head forwards sightly. When he begins speaking in the following shot, he is suddenly standing upright. (00:11:20)

Continuity mistake: When the elderly inmate falls over after being hit with a wooden train, his glasses break off and land on the floor beside him. A few seconds later, when the bald inmate trips him over, the glasses have suddenly rotated 90°. (00:07:53)

Continuity mistake: When the elderly inmate has a wooden train smashed against his head, he is shown positioned in front of the bathroom sink. He then begins to stand up and charges towards the other inmates, and he is suddenly positioned much further back than where he was a few seconds earlier. (00:07:53)

Revealing mistake: When the elderly inmate is pushed over by the other prisoners, a wooden train he made falls out of his clothing. It can be noticed that the man is just holding the train in his hand whilst falling and drops it when he lands on the ground to make it appear as if it fell out of his clothing. He was not meant to be holding the train as it was not shown being held in his hand at any previous point. (00:07:25)

Continuity mistake: The warden attempts to trap Ricky by pushing a table at him. On the table there is a tray filled with meat from when the warden forced an inmate's arm into a meat grinder earlier in the scene. In the shot where he first pushes the table, the meat in the tray is towards the right hand corner of the tray. Ricky then jumps over the table and the meat is now towards the left hand side. (01:26:27)

Revealing mistake: When the warden has finished shoving the inmate's arm into the meat grinder, there is a shot of the warden pulling the inmate away from the grinder and showing him the tray of meat made from his arm. In the shot, we can see that the inmate's face has been covered in white powder to make him appear as if his face has gone pale due to the blood loss, but it makes his face look completely unnatural, and not at all how a human face would ever appear in a situation like that. Revealing mistake: Whilst the cell is filling with cement, Ricky runs over to the steel door and bursts through it to escape. In the shot of him bursting through the door, we can see that the interior of the door is extremely thin and is barely a millimeter deep, revealing that it is a fake prop door and not steel. (00:53:45)

Continuity mistake: When the warden is kicked across the kitchen by Ricky, he lands on a sink. On the floor beneath the kitchen sink, a couple of tiles that were stuck to the wall fall off it. We are then shown a closer shot of the same area, where the tiles that just fell off the wall are now back on the wall, and two entirely different tiles have now fallen off. (01:26:31)

Revealing mistake: When Ricky jumps over the table after the warden pushes it at him, there is a shot of Ricky kicking the warden with his foot whilst in mid-air. In this shot, we can see that Ricky's trousers are being pulled upwards, revealing that there is a wire attached to his trousers that is pulling him into the air. (01:26:30)

Revealing mistake: When the prison wall begins to break apart after Ricky punches it, in the wide shot where the debris from the punch is shown falling to the ground, multiple wires can be seen pulling the chunks of debris from the wall. (01:30:04)

The camera changes angles again and the warden is suddenly facing Ricky directly, then in the next shot, the warden has suddenly rotated 90° again. (01:28:53)
