Revealing mistake: When the Captain is tripped over by Ricky, his face lands on a plank of wood with nails sticking out of it. In the shot of his face getting impaled with the nails, we can see that the Captain has been replaced with a dummy. (00:08:50)

Revealing mistake: In the shot where Ricky punches the prison wall, a split-second before Ricky punches it, a faint outline can be seen to the right of him that gets blasted out of the wall a second later, revealing where the wall was filled in with fake debris and later plastered over. (01:29:59)

Other mistake: Whilst Ricky is fighting Wong Chun, White God attempts to sneak away by getting into one of the dumbwaiters, but the warden shoots him and he explodes. The bullet is shown hitting him in the face, but given the position of White God's body and where the warden was shooting, this would be impossible. When White God crawls into the dumbwaiter, the pantry door is shown closing behind him, so the bullet would have been blocked by the door and would have made it impossible to hit him in the face. Revealing mistake: When the assistant warden explodes after his body starts inflating due to the warden shooting him with the elephant bullets, in the close-up shot of his head exploding, we can see that his head has been replaced with a fake dummy head. (01:23:18)

Revealing mistake: Just after the warden has shot the assistant warden with the bullet made for shooting elephants, there is a shot of the assistant warden's body inflating. In the shot, we can see that really bad prosthetics have been added to his forehead, cheeks, nose and chin to make it appear as if his head is inflating. It is extremely noticeable where the prosthetics end and where his real face begins, making the whole effect appear completely unnatural. (01:23:12)

Continuity mistake: When the warden is thrown across the kitchen and into the meat grinder by Ricky, in one shot, he is shown being thrown across the room from left to right. When he lands in the meat grinder in the next shot, he is suddenly falling downwards. (01:28:37)

Other mistake: Just after the warden tells Ricky they are trained in the same kung fu style, Ricky thinks to himself for a moment. The subtitles read 'If your opponent is strong, use your wits to beat him', but before the start of the line, before the word "If", a randomly placed comma can be seen at the start of the line. There is no reason for this comma to be here since there are no words shown before it. Clearly this was an unintentional error made when the subtitles were being typed out. (This depends on which format and which studio distribution you are watching, but it can definitely be seen on the Region B Blu-Ray distributed by MediumRare). (01:28:10)

Continuity mistake: In the final fight scene, there is a moment where Ricky punches straight through the warden's stomach and punches out through the other side. The shot then cuts to the warden for a second, but when the shot cuts again, Ricky's arm has suddenly been completely removed from the warden's stomach. No way could Ricky have removed his entire arm from the warden's stomach that quickly. (01:27:33)

Revealing mistake: When Ricky attempts to fight the muscular warden, he punches the warden in the stomach and the warden then punches him into a support column. Just before Ricky crashes into the column, a wire can be seen attached to him, pulling him backwards towards the column. (01:27:20)

Continuity mistake: When the warden begins changing into a more muscular version of himself, in one shot, he is shown with his head pointed downwards and with his eyes closed, but in the next shot, he is suddenly standing upright, with his eyes open. (01:26:37)

Other mistake: Wong Chun attempts to kick Ricky whilst flying through the air, but Ricky grabs onto his and throws his leg into the hydrogen tank. The rest of Wong Chun's body then falls to the ground, but the leg is still shown in the exact same elevated position despite the rest of his body being lowered. (01:25:32)

Continuity mistake: When the assistant warden begins to inflate after getting shot by the warden, Ricky and a group of other inmates begin to back away from him and are shown standing in the background. But in the shot of the assistant warden exploding, Ricky and the other inmates have suddenly disappeared. (01:23:15)

Other mistake: Just after the warden tells the guard to limit the amount of food the inmates get in their meals, one of the inmates shown polishing the meat grinder says "You eat like a lousy king. Even your dog gets a big steak." When he says "your dog", the subtitles read "yourdog" instead of "your dog." It has been incorrectly spelt as one word, with no space shown in between the two words. (This depends on which format and which studio distribution you are watching, but it can definitely be seen on the Region B Blu-Ray distributed by MediumRare). (01:21:53)

Other mistake: Just after the assistant warden's right arm has been chopped off by an inmate, part of his shirt sleeve goes missing. The part of his arm still attached to his body has a sleeve starting at his shoulder, and stopping an inch above where his arm stump ends, leaving an inch of skin exposed. And when the chopped off arm is shown, the arm is shown to have a sleeve starting at his elbow, going down to his wrist, but there are a few inches of his upper arm above his elbow left exposed. This means that a couple inches of his sleeve from where part of his upper arm was located, have gone missing without explanation. (01:20:25)

Continuity mistake: When the armoured guards appear, a guard holding a mace attempts to swing it at Ricky whilst he jumps from a staircase. In the shot of Ricky jumping down from the staircase, he is holding his shield so that it is positioned away from him, but in the next shot, the guard suddenly has his protective shield directly in front of him. (01:20:49)

Continuity mistake: When the assistant warden falls down the stairs whilst being confronted by the inmates, in one shot his body is rotated towards the left and is face to face with some inmates nearest to the staircase. When the camera changes angles, his body is more rotated to the right and is now directly facing the staircase bannister to be face to face with more of the inmates. (01:20:16)

Revealing mistake: When the assistant warden and the bald inmate walk into Ricky's cell after not seeing him inside, Ricky reveals himself, grabs onto the bald inmate and punches part of his head off his body, killing him. In the shot where Ricky punches the inmate in the side of the head, we can see that the actor has been replaced with a dummy. (01:19:17)

Continuity mistake: When the assistant warden and the bald inmate are walking down the corridor towards the end, two inmates from inside their cells put their hands on the bars of the cell doors to see what is happening. The inmate standing on the left has both of his hands on the bars and the inmate standing on the right has both hands on the row of bars positioned below where the inmate standing on the left has his hands. When the camera changes angles, the inmate standing on the left no longer has his hands on the bars and the inmate standing on the right now has his hands positioned on two separate rows of bars.
