Visible crew/equipment: When Chucky flips off Nica from outside, if you look closely, for about one second, you can see the curly, auburn hair of a crew member on the right side of the screen ducking behind the actress.

Visible crew/equipment: When the camera cuts to Andy's close-up as he's punching in his safe code, you can see a set light reflected in his eye. There's nothing in the scene in front of him that could be creating such a reflection.

Visible crew/equipment: When the patient throws Chucky into the hole outside, when he rises up, you can see someone's hand give her the doll.

Suggested correction: That's the character Malcolm's hands. He's in the grave. He handed the doll up to her, and then, literally seconds later, the nurses help him out of the grave. I genuinely don't understand how that could be mistaken for a crew member's hand when it's so obviously a character in the film.

Continuity mistake: When Andy is grappling with the "buzz-cut" Chucky and trying to rip his clothes off, at one point the camera cuts from a medium shot to a closeup as he rips Chucky's overalls down. In the medium shot, Chucky's left arm is reaching for Andy's wrist, but isn't holding it. But in the closeup, suddenly Chucky is grasping Andy's wrist.

Answer: Chucky states that he found a new voodoo spell in the film that allows him to control multiple hosts at once. It's almost like a copy/paste of his spirit. Andy still has the original Chucky, whereas the rest are duplicates.

Answer: Originally, the serial killer Charles Lee Ray used a voodoo spell to transfer his soul into a "Good Guy" doll in a toy shop, thereby becoming "Chucky" the homicidal doll. Later he acquired a voodoo spell that allowed Chucky to transfer souls multiple times (of course, his ultimate goal has always been to transfer his soul back into a living human body).
