Revealing mistake: A loud, fierce gun battle rages between dozens of 5-0 sharpshooters, Army MPs and the desperadoes inside the payroll office. Amidst all the high-powered rifle fire, a woman in a blue dress strolls casually by in the background, well within bullet range, and never even turns her head to see what all the ruckus is about. Character mistake: McGarrett tells Pereno that the sabotaged race car went over a cliff. But the car crashed into a wall, with no cliff in sight. It's very unlike Hawaii's top cop to get the facts wrong - especially when he was an eye witness to the event. (00:44:00)

Visible crew/equipment: As the mob bosses gather for their summit meeting, one of their big black Lincoln Continentals reflects several banks of studio lights when it comes through the security gates. (00:16:40)
Twenty-Four Karat Kill - S1-E8
Continuity mistake: When Andrea finishes wiping the test fluid from the gold bar, she holds it out with her left hand. After the cut scene, her right hand is holding the bar.
Revealing mistake: In last scene, the killer, Aarons, is shown being led into court handcuffed. When they then film from the back, you can see his right hand free, and he quickly moves it to the other to make it appear like he really is handcuffed.
Continuity mistake: At 0:48:48 the grave marker is reversed (i.e. mirror image), and when they show the marker again at 0:48:50 it is correct. (00:48:45)
Mother's Deadly Helper - S6-E22
Audio problem: Chin says "Telex from Washington"; Steve says "Good, good". Clearly overdubbed. Watch - their lips don't move. (00:40:20)
A Bullet for El Diablo - S6-E10
Factual error: Pepe and Carlos are driving a '62 Chevy. When they watch Ramos' motorcade pass, there's an insert shot of a key in the steering column as they start the car. In a 1962 model, the ignition would have been on the dash, not the steering column. (00:13:15)
Revealing mistake: Listie was supposed to be dead, but you can see his eyeballs moving under the lids.
Character mistake: While trying to figure out how to evacuate the other patients in order to get to Danny, McGarrett mentions a patient's daughter in room 309, but it's mentioned in other scenes that the man and his daughter are in room 306.
'V' For Vashon: The Father (2) - S5-E10
Character mistake: Hayward tells Vashon that McGarrett's office is on the third floor of the Iolani Palace. Previous episodes have placed the office on the second floor, or have shown the palace to have only two floors. (00:31:45)

Continuity mistake: McGarrett approaches the Governor's office building wearing a light blue suit, but enters the lobby wearing a dark navy blue one. (00:27:05)
Continuity mistake: McGarrett leaves the parking lot driving a two-door Ford Mercury, but arrives at the crime scene in a newer, four-door model. (00:07:25)
The Ninety-Second War (1) - S4-E16
Factual error: An injured McGarrett is trapped in an overturned car, which emergency workers flip over before cutting him free. No rescue crew would ever do this: such violent movement could kill the patient. It's crucial that crash victims be extricated without causing further trauma: McGarrett would be freed leaving the wreck exactly where it was - upside down. (00:07:25)
Odd Man In - S4-E14
Continuity mistake: At the end, Filer puts his jacket on, and the left side of his shirt collar is sticking out above the coat. Every time the camera angle changes, though, the shirt collar is neatly tucked in. (00:49:00)
Answer: He was fired. He never really appreciated his character. He felt Kono was portrayed as a big dumb Hawaiian and that the stereotype was racist. He also felt underutilized. He was fired after a heated argument with the show's publicist regarding his character. It seems there's not much details given regarding the incident or the firing, so it's seems possible he upset the show's producers as well.
