Continuity mistake: Ruzek and Hailey are waiting to swap cash for guns. The pickup truck drives up very close to them and stops but then when the driver exits the truck he is parked a considerable distance away.

Continuity mistake: While Rojas and Upton are questioning their suspect in the interrogation room, the plate with a sandwich is constantly shifting position from near the detectives to immediately in front of the suspect with no-one touching it.

Continuity mistake: Atwater and Rojas begin to chase a suspect out of the library where he had been spotted by a uniformed officer. As the chase begins Atwater moves ahead of Rojas with the officer in a distant third place. As they emerge from the rear door Rojas exits first, followed by the officer with Atwater behind him by several seconds.
Character mistake: When Ruzek and Voight are questioning Wade Henslow (the father of missing girl) at the station, after finding out he lied and set up his own security to find his daughter Adam looks at Wade Henson but addresses him as "Mr. Larson" which is actually the last name of one of the kidnappers they found DOA who got into the gun fight with his security guy, the ex-cop Kenny. Factual error: Adam is shot, and Kim responds to give him aid. Kim has her firearm out, searching and asking Adam, "where is he?" Instead of holstering her firearm, she tossed it aside to help Adam. Her action is not in accordance with law enforcement training. (00:36:17)
Continuity mistake: Season 9 carries straight on from Season 8 - it is the morning after Kim's shooting. The previous night, they were wearing coats and their breath was steaming. Now there is sunshine, green leaves on the trees, and everyone is in T-shirts.
Factual error: Kevin sleeps overnight at woman's apartment. He doesn't share that he's a cop. The next morning in his unmarked police vehicle he uses his handheld radio that was appeared to be stored in the console and shows up on a violent scene with his weapon. It's obvious that Kevin used his official vehicle for personal reasons, and stored his radio and weapon in the vehicle unprotected overnight, which every police officer knows is wrong and a violation. Plot hole: Kevin is keeping his true identity that he is a police officer from Celeste while Intelligence is investigating a murder committed by one of her art students. Although Kevin is hiding his identity from her, he participated in the daylight search of her art studio looking for the murder weapon. How was Celeste not present during the search of her studio or not aware of the search happening without Kevin being revealed as a police officer? (00:54:04)
Continuity mistake: When Atwater finds the gun in the box, it's sitting on lined notebook paper but when Voight looks in the box, the gun is now sitting on cardboard.
Plot hole: Voight and Trudy are sitting in Voight's patrol vehicle discussing Roy Walton and Mark Irwin. They state that Intelligence killed Mark Irwin. After, Voight is shown checking his vehicle for a tracker and bug, which he finds on and in the vehicle. Based on Voight's experience, he should have checked the vehicle first. Also, the case should be done, as the FBI would have heard their conversation. (00:31:03 - 00:32:05)
Continuity mistake: Toward the end of the episode Burgess begins to board the bus looking for the murderer. As she boards she is greeted by the bus driver who is sitting behind the wheel but as she begins to walk down the aisle the driver's seat is empty.

Continuity mistake: Upton finds the young girl in the warehouse and while she is comforting her, the perp sneaks up behind her and knocks her out with a blow to the back of her head. The injury, however, from that point on, is on her forehead.
Other mistake: Upton needs sugar and asks Attwater if he wants some. Despite the fact that he says yes, she heads to the vending machine and buys one candy bar.
Other mistake: S12 E2 Blood Bleeds Blue - Dante drives by the Green Line Morgan station stairs. It's the same stop Voight drives by as the stairs to the Green Line Kedzie station. The views are from a slightly different angle, but both station stops have a bike, a green scooter, the same weed growth, and a blue recycling bin in the same positions. (00:44:49 - 00:47:08)
