Pro-Dueling - S2-E38

Other mistake: When Alexis activates her Ritual Weapon spell, her Cyber Angel Benten gains 1500 attack points. When this happens, the Life Point display is shown to display the Benten's attack points going from 1800 to 3300, instead of the Attack Point display that should've been shown. (00:13:40)

Tough Love - S2-E43

Other mistake: When Zane and Aster are talking outside the Duel Academy building, in the shot of Aster saying "I'm just impressed that I beat you on live TV", Zane's deck can be seen inside the duel disk he is wearing. The top card doesn't appear to have the black-coloured oval shape that is usually shown on the cards. (00:02:40)

The Dark Light - S2-E45

Other mistake: When Dr Collector activates Graceful Charity for the second time, he has it in his right hand. He then appears to lower the card off-screen, but then he goes to draw three cards from his deck with his right hand. He never placed Graceful Charity on his duel disk or sent it to his graveyard. It's just never seen in his hand again. (00:12:24)

Ultimate Destiny - S2-E46

Other mistake: When Aster summons Destiny Heroes Diamond Dude and Captain Tenacious to the field, they are summoned in defence mode, but on the points display, the word ATK is shown, indicating that it is in attack mode. (00:14:42)

Future Changes - S2-E52

Other mistake: When Jaden summons Elemental Hero Neos using his Silent Doom card, Neos is in defence mode when it appears on the field, but on the point display that appears when a monster is summoned, the word ATK is listed, meaning that it is in attack mode, when it should say DEF (defence mode). Tag Team Trial: Part 2 - S1-E11

Other mistake: Syrus summons to the field Drillroid and has it attack the Gate Guardian in defense mode, but is blocked by Defence Wall because of its effect, and Defense Wall is destroyed. Because of this, Jaden and Syrus lose 500 Life Points. While it does seem logical that their life points would go down because Drillroid has 1600 attack points and Defense Wall has 2100 defense points, they should not lose life points in the first place. This is mainly due to the fact that Drillroid's effect causes a defense position monster to be destroyed instantly, without losing life points. If Drillroid was capable of destroying Defence Wall, then their life points should have remained at 1700. (00:08:00)
