When West is backing up into the room, as the Marquis says, "Boys!", one of them is now descending down from the top of stairs - no one is lying on the steps or getting up from steps. (00:31:15)

Continuity mistake: Halfway through this episode, West's hands are tied to the bedpost. He frees a hidden knife from his lower right sleeve. The close-up of his right wrist shows two rope strands binding him. When West finally gets the knife in the position to cut the rope, only one rope strand is wrapped around his wrist.

Continuity mistake: Towards the beginning of this episode when West is first taken to see the commandant, Cochon goes to the rack on the wall to get West a clean shirt. They are standing by the door where Cochon throws the dirty shirt. There are three pegs on the rack, with a shirt hanging on the leftmost peg and rightmost peg. Cochon gets the shirt off the one on the right peg. As they walk away, West's dirty shirt is now on the ground under the left peg. After the meeting with the commodant, West and Cochon leave. Cochon asks for the clean shirt that West is wearing. When Cochon goes back to the rack to get West a replacement shirt, the shirt that was on the far left peg is now on the center peg, but West's dirty shirt is still under the left peg.

It stays that way for the rest of the scene - as he is shaving, after he is drugged, as he is shooting at Dr. Loveless, as he looks out the window at the fallen Loveless, and as he runs out of hotel looking for Loveless. While West is still outside, Gordon arrives, and West's hair looks as if has gel in it when he is talking with Gordon. After the first freeze frame, West's hair goes back to the non-gel look.

The camera angle from above shows him standing on a pail with his arms extended upward - so he can hold on to the beam - as he is waiting for the fuse to thrust him upward. At the moment the fuse propels him upward, the close-up shows West's arms down at his side, but as he is ascending towards the beam his arms are extended upward.
