Factual error: During the cutscene on the Russian freighter while Steiner explains Dragovich's sleeper cells, a soldier is firing off rapid photos with flash. The camera resembles a 1960s 35mm rangefinder, which would have required manual film advance and single-use flashbulbs or a slow capacitor recharge. Such cameras of the era were incapable of continuous, strobe-like flash photography, making this depiction inaccurate.

Audio problem: In the 1945 flashback, when Reznov is talking at the start, saying "Yes, Dimitri... Time to hunt down the last remnants of the fascist Reich," his head is bobbing up and down as he speaks, but his mouth is not moving.

Trivia: In the Pentagon, when Kennedy is walking up to Mason, Mason has a brief hallucination, and the TV screens in the background change to blink-and-you-miss-it moments during and after the IRL Kennedy assassination. Footage of Kennedy addressing the crowd, getting into his convertible Lincoln Continental, his mourning wife Jackie in her funeral garb, and Kennedy's coffin draped with the American flag, as well as a mugshot of Lee Harvey Oswald.

Trivia: When walking through the Pentagon, if you look to the left of one of the clerk's desks, you can see an Easter egg for Call of Duty: World at War. A picture of a battle and a plaque that reads "Battle of Peleliu: Operation Stalemate 1944."

Revealing mistake: If a player has a rocket launcher on their back as a secondary weapon, sometimes after getting killed, the death animation has the character throwing his head back. The back of the long rocket launcher tube goes into his head.
