Continuity mistake: "Nationwide Airways" is the name of the fictional carrier used in the episode, and it even has its own logo and livery. Yet in the exterior shot of the plane pushing back from the gate, it has suddenly transformed itself into a Delta Air Lines plane. It also looks like a 1990s Delta plane while it is taking off. Continuity mistake: The edition of the San Francisco Chronicle with the headline on Jimmy Cusack's death is dated April 1, 2006. However, at the end of the episode, which is a few days later, the newspaper with the front page photo of Stottlemeyer arresting Ron Neely, is dated March 24, 2006. In addition to the fact that the date of the arrest is a full week before the date of the murder, April 1, 2006 fell on a Saturday, when the episode says the murder was on a Monday night. Factual error: The newspaper with the front page headline about Jimmy Cusack's death comes out the same morning that he is found dead. In reality, since it's said later that the cleaning crew found his body around 7:00 AM, it would be way too late for his death to have appeared in that same morning's edition of the San Francisco Chronicle. It would more likely appear in the next day's paper, by which time Monk had found out that Cusack had been murdered. (00:05:05)

Continuity mistake: When Monk and Natalie are getting out of their car at Max Hudson's house, there is a woman on the sidewalk walking past the house. As soon as Monk asks, "What station is he on?" the camera angle changes to a view from Max's porch, and said woman suddenly disappears, when she should be seen walking across the driveway. Factual error: While Monk and Natalie are waiting for the mayor to exit City Hall with his aide, you can see a cable car turning around the corner behind them. San Francisco City Hall is located on Van Ness Avenue between Grove and McAllister Streets, and this is at least six blocks from the nearest cable car line (which would be the turntable at Powell and Market Streets). Continuity mistake: When Natalie is noticing Monk's nickname on his registration form, you can see a female extra with black hair, a gray outfit and a purse on her left shoulder walk past Natalie on the left side of the screen. Twenty seconds later, as Natalie is turning and preparing to walk away, the exact same female extra walks past her again, again going in the same direction. Also, in the same shot, you see a male extra with a satchel over his shoulder walk past the camera as Natalie is pulling the form from her purse, and he briefly and instantly reappears walking past Natalie again at the point where she grins and says, "I don't know!" for the second time. Continuity mistake: When Monk and Natalie are first talking to Kendra Frank, when Kendra says,"Look, they're trying to say he OD'd! Okay? That's impossible! He's been clean for 17 months! I know, I talked to him about it every day!" you can see that Monk has his right arm down by his side. But when the camera changes angle to Natalie replying,"Well, Kendra, we were there. We saw a needle in his arm," Monk's right hand goes from being by his side to scratching the side of his cheek with his finger. (00:13:10 - 00:14:35)

Revealing mistake: When Reese opens fire on Adrian, Jack and Natalie at his house, in a shot from Reese's perspective, the squibs from bullet hits on the ground appear to be below the barrel's trajectory. The bullets should be hitting the ground closer to his targets' feet.

Factual error: In real life, the FBI does not have jurisdiction over routine homicide cases, which are almost always left to the local police. They have no jurisdiction over serial murder at all, unless a federal employee is a victim or one of the murders gets committed on federal property (like an army base, federal prison or national park, or on sovereign territory like a reservation). The only time the FBI would be involved in a homicide is if the individual crossed state lines in committing the crime.

Continuity mistake: At the start of the episode, when Katherine Rutherford is killed, she lands on her stomach with her eyes wide-open. Yet when Stottlemeyer and Disher arrive and see the cops looking at the body, she is now lying face-up and her eyes are closed. It is hard to believe that the police would move the body that much without first marking its original position with tape.

Plot hole: How come Monk and Natalie didn't think it suspicious that Cassie Drake was killed in her house within less than twelve hours after they had found out she was responsible for killing her patient? Since Jake and Ramone were the only two people they had disclosed their findings to (apart from Stottlemeyer and Disher), they should have focused in on Jake much sooner. (00:24:00 - 00:27:00)

Continuity mistake: There are two noticeable continuity errors during the police investigation in Cassie Drake's house. When Stottlemeyer is squatting next to Monk, Monk's right hand instantly changes position between shots. Also notice that there is no sheet over the body at the beginning of the scene, but a few shots later, suddenly the lower part of Cassie's body is covered with a sheet despite no forensic technicians being there to place it. Continuity mistake: There are two mistakes when Natalie runs up to Monk after he leaves the port-a-potty. First, notice that when Monk is replying to Natalie's first question, her thumb is sticking out, but it isn't when the shot cuts to a front shot of Natalie. Secondly, as Natalie is leading Monk away, her purse has gone from being over her shoulder to being over her elbow, and back when Monk and Natalie turn to see Stork's body fall out of the port-a-potty. Plot hole: Monk and Natalie temporarily incapacitate Jake before crawling all the way down the hall from the bathroom to the living room while pulling a heavy claw-footed bathtub. Exactly why didn't Monk or Natalie think to consider a more permanent solution to preventing Jake from attempting to kill them again, like grabbing his gun and using it to detain him until the police could arrive to arrest him? Continuity mistake: The firehouse's number is 53, and is seen on the various fire trucks throughout the episode. However, in an establishing shot at the opening of the episode, you can see letters for "Eng. Company 1" and "Truck Company 1" over the garage door. This is because Fire Station 1, located in Los Angeles, was utilized for filming.

Factual error: Commander Whitaker tries to drown Monk and Natalie by trapping them in the ballast tank of the submarine. The ballast tanks on a submarine are inaccessible from the interior, and wouldn't have dramatic lighting in them either.

Continuity mistake: The lighting and position of the sun changes constantly throughout the episode and between scenes. For example, when Monk and Natalie are at the first aid tent, it looks cloudy and the lighting suggests that it is midday. Yet in the very next scene, when Monk, Natalie and Kendra catch up to Stottlemeyer, Disher and Jared, it appears to be sunny and late afternoon/early evening. (00:19:00 - 00:25:05)
