Revealing mistake: At the beginning of the film during Bond's "funeral", the shot cuts to a man watching the funeral from a boat through a pair of binoculars. The camera then moves to the newspaper next to him showing an "article" reporting Bond's murder. If you look closely at the "article" itself, you can see it was actually pasted onto the actual local newspaper. They probably bought a local newspaper and pasted the fake article over it to make it look authentic.

Plot hole: Right after Bond escapes Blofeld and joins Tanaka and Kissy, they say they need to get to the control room (that Bond just left from). They look at the "front door" to the room, and Tanaka says, "Impossible! It's too well defended!". Why did they not go back the way Bond just came from, through Blofeld's "apartment"? Instead, they fight their way across the entire launching complex and go through another entrance, which also leads through Blofeld's apartment.

Revealing mistake: During the car chase scene, if you watch the speedometer on the car, it never rises above zero. That, and the obviously poor image quality of the "road" in front of them, shows that they were using projected film in front of the car.

Continuity mistake: When Bond is attacked by the ninja while he is training to be a ninja, Bond grabs the staff with the knife at the end, flips the ninja and stabs him on the left side of his lower back, but when it cuts to the next wide shot, it shows all the blood on the front of the ninja's training top. The only way there should be blood there was if the blade penetrated from the back and out the front, but the blade wasn't anywhere near long enough to go completely through a body.
