Continuity mistake: In the scene where Stallone is chasing and shooting at the killer in the factory, he bursts through a door and shoots the hanging body of the FBI/SWAT officer. In the first shot of the officer you can see his lips. The next shot of the body shows that his mouth is duct taped.

Other mistake: In the scene where Stallone's partner is driving him to the detox center, you first see the truck with tyre chains on only the rear wheels. Half a minute down the road they are on all four tyres.
Factual error: No way the sound of Jenny's voice calling for Malloy from afar through Slater's walkie-talkie was this loud and with no noise at all (at Malloy's end) from the storm she and Slater was in.
FBI Agent Jake Malloy: You're real good at kicking them when they're down, aren't you?
Noah: I do all right when they're standing up, too.
FBI Agent Jake Malloy: You never stood up to anybody.
Noah: Yeah? Well at least I'm there for my women.
Slater: Can't sleep? Didn't mean to spook you.
FBI Agent Jake Malloy: I never knew.
Slater: Knew that you could miss someone so much?
FBI Agent Jake Malloy: I didn't know that.
Slater: The thing you have to remember, Malloy, is that sometimes the really bad things in life can make you stronger.
