Continuity mistake: When Jeff finds the second box (after the freezer room) he takes the card and flips it over to see the picture on the back. In the closeup his thumbs are on the bottom corners, but then the shot cuts away to a view from his left and you can see that his left thumb is at the top left corner and his right thumb is as least halfway up the card (instant cut, no time to move his hands). Continuity mistake: When Lynn places down the drill there are 4 pieces of a broken mirror on the table in front of her. The 2 middle ones are standing straight up, but the outer ones are at various angles of slant. The next shot (a few seconds later) all 4 pieces are standing straight up and arranged very nicely. Continuity mistake: The tear in Jeff's face (after he gets stuck to the refrigeration machine) continually changes throughout the movie. It goes from oozing and bloody, to clean and almost normal looking several different times. There's no blood on his bathrobe, so he didn't wipe it off that way. That's a big enough tear to bleed a decent amount, so we'd see the blood if he wiped it off. (00:52:35)

Continuity mistake: When the tape player says "The key is among your son's possessions" Jeff is standing in front of the opening between the bars leading down to the ladder, and he takes a few steps towards the incinerator (his left). The screen cuts back to Jeff and you can see in the lower left-hand corner of the screen that Jeff is still standing in front of that opening between the bars. (00:56:10)

Continuity mistake: In the "pig" room the string around the tape player suddenly disappears in the middle of the tape message. Jeff's right hand never changes position and the string was tied around the middle of the player, he would have had to move his hand to get it off. (00:58:00)

Continuity mistake: There is a far-away shot when the first pig comes out. You can see Jeff standing on the platform holding his left hand to his face, there's an instant cut to a close-up of Jeff, but now his right hand is held up to his face. (00:58:45)

Continuity mistake: In the 'Pig Vat' scene, the slide that the pigs slide on into the blades is clear with no pigs stuck on it. The pigs fall in one after the other with none getting stuck on the slide. However when the fourth pig drops onto the slide, you can see the a pig stuck on the slide and hanging over the edge of the slide. The next couple of pigs that fall don't knock another pig in and you cannot see the pig overhanging on the slide. (00:59:25)

Continuity mistake: Lynn explains the procedure she's going to do to Jigsaw, then goes to fill the syringe. Look at the syringe when she sticks it in the bottle, the plunger part is pushed all the way to the bottom. The shot cuts to a different view and the plunger is a good inch or 2 pulled back already (she hadn't started to draw it out yet, you can see 1 or 2 inches of air). The shot cuts again, and now the plunger is at the top of the syringe again, then she starts drawing it out). (01:01:30)

Continuity mistake: When Lynn is operating on John's head, she the skin from the skull and then clips off some veins. Right when she asks Amanda to fill the syringe with alcohol to clean John's head, Lynn turns back to wipe up some of the blood, but there are no clips in place. A shot after that, we see her taking the syringe from Amanda's head and squirting the alcohol on John's head and the clips are there. (01:03:05)

Continuity mistake: When Lynn cuts open Jigsaws head, she clips off some veins with clamps and lets one fall in front of his face. A scene change to the front of his head shows no such clamp lying across his face. (01:03:05)

Continuity mistake: The blood splatters on Lynn's face change throughout the whole brain surgery scene. The splatters go from defined, to smeared, and then back to defined again a couple different times. (01:03:15)

Continuity mistake: The amount of blood on Jigsaw's skull changes drastically from thick dark red to thin almost translucent red. This is before Lyn clamps off the blood vessels and uses the alcohol to clear the area. (01:03:20)

Continuity mistake: When Jigsaw is in the process of passing out (after surgery) Lynn's hands move off the bed, back on, then off again, then back on (it wasn't just her moving them naturally, they were in the EXACT same positions each time they were back on the bed). (01:04:00)

Continuity mistake: The drill that Lynn uses on Jigsaw's skull has a white piece of tape on the end of the drill bit a short distance from the end, presumably to mark the thickness of his skull so she knows where to stop. In one shot (during the drilling scene) the piece of tape moves a good two inches farther up on the drill bit, but it's back down in the very next shot. (01:04:05)

Continuity mistake: In the close-up shot of Lynn removing the piece of Jigsaw's skull you can see that she's holding the instrument in her left hand (look at the position of her fingers), but in the farther away shots she has it in her right hand. (further evidenced by later in the scene where you see a close-up and she's still holding the instrument in the left hand and a knife is in her right). (01:04:30)

Continuity mistake: When Amanda takes some items out of her box, she lays a red cloth on the bed and then places some items on it. She places a razor blade down on the cloth and there is a large crease underneath the blade. Cut to another shot and the cloth is completely smooth; no crease. (01:07:50)

Continuity mistake: Amanda's sweat marks and eyeliner runs constantly change shape and size (bigger, smaller, and back to bigger) during the bathroom flashback. (01:10:30)

Trivia: The camera moves from Troy's crime scene to Kerry in the bathtub in one shot - Dina Meyer had to run around the set, undress, and jump into the tub. If you look closely, you can still see the water moving from when she jumped in.
