At the end, the one-armed Miguel uses the lift and opens the gate restraining the zombies. Before he is ripped apart, he operates the lift and lets the zombies into the facility. Rickles head is ripped off, and Rhodes abandons Steele. Steele locks himself in a room but is quickly overtaken by zombies. He shoots himself in the mouth. Rhodes is ambushed while opening a door and is torn in half by zombies. Sarah, John, and William climb up a silo and reach the helicopter with the zombies close behind. As Sarah enters the helicopter, she is jumped by a zombie but she wakes up on an island with William and John there with her. She takes out a calendar and crosses out the date.

Trivia: Joe Pilato who plays Captain Rhodes in this film had a small role in the Director's Cut of its predecessor, Dawn of the Dead: he's the police captain at the dock scene.
