Continuity mistake: When Mai embraces Ando as he cries in the courtyard while talking about his fear of death, the way Mai has her hands around his head repeatedly changes between cuts. For example, she moves her hand from the back of his head to his cheek twice between cuts. Her hand goes from his cheek to closer to his mouth between cuts. Etc.
Continuity mistake: After Ando destroys the second tape in the courtyard, as Mai walks up to him, the way he's holding his hands on his knees changes between cuts. In the wide shot, his wrists are on his knees and his hands are dangling in front; in the medium shot, his hands are gripping his knees. Mai also gets significantly closer to him between cuts, as indicated by her shadow.
Continuity mistake: When Yoshini drops dead in his apartment, as Ando runs up to him, the papers scattered on the floor around Yoshini change between cuts.
Continuity mistake: When Ando goes to visit Yoshini at his apartment, Yoshini stumbles away coughing. His left arm is outstretched and gripping the wall. Instantly, in the next cut, as it punches into a close-up, his left hand is up, grabbing near his neck.
Continuity mistake: After Ando watches the tape, he has a vision of Sadako appearing nude and initiating a sexual encounter. When she grabs his hand, it goes from being right against his face to about six inches away between cuts.
Continuity mistake: When we see the flashback of Sadako's death, it plays out differently than in the first film. In that film, Sadako calmly walked to the well, only for the doctor to sneak up behind her and strike her while she is unsuspecting. In this film, she is fleeing in terror from him and turns to see him coming.
Continuity mistake: After getting the tape, Ando goes back to his apartment and starts to figure out the coded message Ryuji left. The way he holds the baggie with the strip of paper in it against the table changes between cuts. In some shots, his pointer finger is on it; in others, his thumb. Etc.
Continuity mistake: When Yoshino leaves the lecture hall after giving Ando the tape, the way he's holding his suitcase and the way Ando is holding the video instantly changes between edits as it cuts to the high-angle wide shot.
Continuity mistake: When Yoshino points out the video of the glowing rats in Ando's lab, you can see one of the doctors handing something to another in the background. It then cuts to a close-up and they're doing something completely different.
Continuity mistake: When Ando considers suicide a second time after having the nightmare, the angle at which the photograph, envelope, and paper are sitting in front of him changes between cuts. They go from pointed straight and parallel to angled and overlapping.
Continuity mistake: When Ando's assistant picks up the photograph of the tumour after Ando explains that it caused Ryuji's death, the way he holds the photograph changes between cuts.
Continuity mistake: When Ando returns to the hospital to explain that a tumour caused Ryuji's heart attack, as his assistant exclaims "A tumour?!", Ando's right hand goes from grasping the side of his bag to pulling a file out from inside it instantly between cuts. Between the same two cuts, his assistant also walks behind him twice from right to left.
Continuity mistake: When Mai has the vision of Ryuji's death, the position of his hands on the walls changes slightly when it cuts to the close-up shot. Most noticeably, his right hand is about 6-8" lower on the wall in the second shot.
Continuity mistake: When Mai returns to Ryuji's apartment near the beginning, she's standing about 2 feet from the pillar to her right. When the camera cuts to the opposite angle, she's suddenly only about 6 inches away from it.
Continuity mistake: The coded note found in Ryuji's throat changes slightly. When it's first found, there appears to be an asterisk next to the first four, while there's about 1-2mm of paper to the side of the eight before the paper tears off. When we see it later in Ando's apartment, the asterisk is missing and the tear is right at the edge of the eight.
Continuity mistake: When the detective grabs Mai, shakes her, and shouts near the beginning, Mai goes from looking downwards to looking straight forward between cuts.
Continuity mistake: When the detective is interviewing Mai with Ando near the beginning, Mai turns her face and the detective grabs it and turns her back. In the first shot, his hand is against her chin, but in the next shot, it's against her cheek.
Continuity mistake: When Mai is first introduced, the detective asks her to explain to Ando how she found Ryuji's body. As this happens, Ando goes from looking down at Mai to sideways at the detective between cuts.
Revealing mistake: When Ando has a vision of Ryuji waking up on the operating table and tilting his head up to look at his hollowed-out body, you can see where the fake prosthetic body and neck are blended with the actor's face, and even see a little bit of the actor's real neck beneath his head.
Other mistake: It makes no sense for Ryuji's head to be surrounded by so much blood in the autopsy room. We're shown the table is built so the blood drains down, so why is there such a massive pattern of blood around his head, on the sides and railings, etc.? It almost looks like blood was spraying everywhere... that doesn't happen in autopsies.
