Revealing mistake: When Ando makes the first incision into Ryuji's body during the autopsy, it's very clear that the skin is a fake prosthetic made of some sort of soft, spongy material. The knife glides into it unrealistically easily, almost like a hot knife through softened butter.
Other mistake: When Ando begins the autopsy, he glances up and says, "Shall we begin?" to someone. However, in the next shot, the other doctor is to the side of him and not in the direction he was looking.
Continuity mistake: When Ando steps into the room to perform the autopsy as the body is being brought in, the two doctors in white coats go from being on the left side of the room to behind Ando in the doorway between cuts. Another pathologist also appears behind him between cuts.
Continuity mistake: When Ando takes the picture of his son and the folded piece of paper with his hair out of the envelope at the beginning, the way he's holding the photo and paper changes slightly when it punches into a close-up. (Most noticeably, the paper goes from poking out from behind the photo to totally behind it.)
Continuity mistake: In the beginning, when Ando sets down the metal box, in the first shot he's holding it with the hinged side facing towards him and against his thumb. When he sets it down, it has turned 180 degrees between cuts, and the side that opens is facing him and against his thumb.
Continuity mistake: In the opening, when Ando grabs the envelope from between the two books, the angle at which he's pulling the left book aside changes between cuts. It goes from being almost straight up to suddenly being at a near-45-degree angle.
