Continuity mistake: I know that Animators re-use backgrounds but it is painfully obvious that when Keitaro and Narusegawa are leaving the exam results posting they walk in a straight line past the same "Ristorante" three times.
Visible crew/equipment: At the beginning of the episode when Libby and Wade are talking, a boom mic can be seen in a mirror that is on the refrigerator.
It's All about the Money, Honey - S6-E10
Continuity mistake: Kelsey stands up to leave the bar with her phone in her hand. After she leaves, her phone is on the table in front of her now empty chair. Next scene she is in her hotel room using her phone.
Factual error: Pierre Bezukhov's spectacles are of the type with nose pads, which were not invented until the late 1920's.
Episode #1.1 - S1-E1
Factual error: There is a view of the night sky in southern Africa. However, both the Big and Little Dippers (along with Polaris) are visible. In general, these constellations are not visible in the southern hemisphere, and where they are (North of 1° South latitude) they are very close to the horizon. The Big Dipper (Ursa Major) is to the left of the screen and Polaris (part of the Little Dipper = Ursa Minor) in near the top-center. This occurs shortly after the lion cub scene. (00:44:00)
Factual error: Emily is in Paris when her boyfriend calls from Chicago and wakes her. She asks the time which he says is 7pm, and she says 3am in Paris, which cannot be. Central European Time is 6 hours ahead of NY and 7 from Chicago. 7 pm in Chicago would be 2 AM in Paris.

Continuity mistake: During dinner, when Sharon hears the kid's laugh she recalls her dead baby. Her left arm is either raised or lowered depending on the shot.
Onna no koi wa snesouyo! Kakutou shintaisou de iza shoubu - S1-E12
Other mistake: When Akane and Ranma arrive to Furinkan High, it's 8 past 20. When Kodachi leaves, it's 8:35. 15 minutes is way too long of a time to have passed (and everyone would also be late for lesson). (00:01:50 - 00:04:40)
Continuity mistake: When Larry Bird hands Marty the pie, Marty takes it with one hand and then grabs it with his other hand. In the next shot, the pie is no longer in his hands and he's seen grabbing it with both hands at once.
Character mistake: In Season 5, Episode 4 ("The Heart Attack Boys"), Abishola twice gives Christina an injection without swabbing her skin with alcohol. Abishola is an RN and would never make that mistake.
Continuity mistake: At the subway station after Miranda says "a Kindle would not have had the same impact", the teacher's left arm suddenly swaps from lowered to raised.
Deliberate mistake: Probably best for filming the scene, but Jennifer Hart pulls up and parks her car on the street on a red curb in front of an ambulance and walks away. (00:10:12)

Continuity mistake: Episode 1 - When the commissioner kisses his wife at the beginning of the first episode, he is holding the holstered gun in his left hand and his ID in the right hand. That's in the first shot, because his daughter enters the room bringing coffee, and now the two items swapped hand. (00:05:10)
Revealing mistake: 44/34 Nick Newman is hit from behind and knocked unconscious. However, he bows his head and braces for the blow before he is hit. (00:48:00)
Factual error: LtCol Baisley has his name and U.S. Army patches on the wrong side of his uniform. He has the Army patch on his right side instead of on the left. (00:33:00)
Other mistake: There is a countdown on the movie screen. It is then reflected in Dusty's glasses. But the numbers are not reversed in the glasses. (00:41:00)
Episode #5.7 - S5-E7
Other mistake: Two actors are standing on an ancient muddy track which shows the tyre prints of a modern vehicle, made probably by the 4WD camera car. (00:09:00 - 00:10:00)
Continuity mistake: After Melissa and Rhonda get off the phone, the way Rhonda is holding her phone changes between shots, and the phone screen changes from on to off.
