Revealing mistake: When Frankie and Tanya visit Eugene while he's playing the video game, he refuses to give them the pills he made. They call him a coward then leave, and in the next closeup of Eugene's video game screen we can see his reflection, but the problem is his reflection is not backwards as it should be (note his shirt buttons, etc). Revealing mistake: When Morgan stabs the walker which is hanging from The Tower, the blade enters its lower head, but the blood can be seen coming from above the hairline. (00:25:30)
Suggested correction: I can see where the confusion is. If you watch it normal time, it looks like he stabs him in the cheekbone. But in slow motion, you can see the blade go into the top of the skull.
Continuity mistake: The hole in the wall Carol makes with the knife grows, depending on what angle we see it from.
Continuity mistake: Daryl and Carol are walking to an abandoned home. Carol walks by a bush with bright sunlight coming from the left of the shot. The angle changes and it's overcast with no sun. Then we see Daryl again and it's once again sunny.
Factual error: The microscope in the CDC lacks objective lenses. (00:36:27)

Continuity mistake: After talking with Hershel, Rick heads out to the barn and meets up with Shane at a tractor. Shane asks him which way is it going to go as Rick is looking to his left down at the tractor. When the shot cuts to The Other Side as Rick responds, he's suddenly looking straight ahead at the barn. Continuity mistake: Rick is talking with Hershel in his kitchen and asks him to please not send Them out into the world again. Hershel is looking down at his sink from the side angle. When it cuts to a view from behind, he's suddenly looking up to the left out of the window, only to lower his head back down towards the sink again. (00:13:57)
Bloodletting - S2-E2
Continuity mistake: When T-Dog is telling Dale that all he found was ibuprofen and cigarettes, he is holding his hand up and they are both looking at each other. In the next shot, suddenly his arm is down and they are both looking forward. (00:22:44)
Continuity mistake: When Aaron and Daryl are stuck in the van surrounded by walkers, he lights up a cigarette with a yellow filter and it changes to a white filter as he starts smoking it.

Revealing mistake: When they pick up Glenn's body, for a brief moment, you can see his head is fully intact.
Chupacabra - S2-E5
Plot hole: How is it possible that Glenn did not see the missing Sophia in the barn full of walkers at the end?
Other mistake: When the Alexandria Group are at the Hilltop trying to convince Gregory to join them in their fight against the Saviors, we can see the actresses who play Michonne and Maggie trying to hold in their laughter. (00:06:02)
Continuity mistake: Daryl is caught in a classroom and fights one of the Pope's men. He has him in a corner by the door where a walker comes. The walker gets killed by people on The Other Side of the door and it noticeably splatters on the glass. Two people break the door down and get inside. The splatter disappears on the glass.
Continuity mistake: When Carl gets shot, and we see him in the ground, not only does the blood spot on his short change in the next shot of him on the ground, but he's in a completely different shirt. It's still a cameo shirt, but the cameo pattern is different.
Factual error: Rosita picks up a spent cartridge fired from the desert eagle earlier in the episode, which is either a .357 Magnum, .44 Magnum, or .50 AE as those are the only chambers that the desert eagle is made in. She then asks Eugene to make her a bullet. In the following episode he does and she uses that bullet to fire a pot shot at Negan. But the casing that she picked up would not fit the 9mm Beretta she uses, nor could it be modified to do so.
