Character mistake: Near the end of the episode, McGarrett tells Dr. Kuh "The imposter Lewis Shen murdered your uncle." The uncle McGarrett is referring to was, earlier in the episode, referred to as Dr. Kuh's grandfather. (00:27:40 - 00:46:45)

Character mistake: To get on board a private yacht, Carl Swanson poses as a repairman from Mauna Loa Refrigeration Company but the name on the back of his uniform is misspelled "Muana Loa."

Rest In Peace, Somebody - S4-E10

Character mistake: Cameron's attempt to assassinate the governor fails because McGarrett has everyone wear bullet-proof vests. It's been established that Cameron is brilliant, a top-of-his-class ex-cop and a crack shot. And he's been sending death threats, so he knows they'll expect him. Yet he fires at the governor's back and is shocked that the man doesn't die. Anyone with his expertise would have anticipated the vests, and aimed for the man's head. (00:48:20)

Character mistake: McGarrett tells Pereno that the sabotaged race car went over a cliff. But the car crashed into a wall, with no cliff in sight. It's very unlike Hawaii's top cop to get the facts wrong - especially when he was an eye witness to the event. (00:44:00)

'V' For Vashon: The Patriarch (3) - S5-E11

Character mistake: When Ben talks to the old guy about Spider Brown, Ben says, "Somebody that got rich the past week." The time between the shooting and trial was probably months, not just one week. Spider would have been flashing money right after the hit, and then disappeared.

The Clock Struck Twelve - S5-E12

Character mistake: After distracting the guards, Danno tosses a grappling hook onto a ledge and climbs the side of the building. He then goes about his business, carelessly leaving the hook and rope dangling from the ledge for all to see. For a seasoned 5-0 team member, that's pretty sloppy covert operations. (00:22:40)

Character mistake: McGarrett mispronounces his old friend Cargill's name in the bank vault scene, giving it a soft G and calling him "Car-jill." Earlier in the episode, and in all the scenes after this, he correctly says "Cargill" with a hard G. (00:17:00)

Trivia: Danno was played by Tim O'Kelley in the 5-0 pilot. But a NY test audience told CBS they didn't buy him as a serious cop because he was too much of a "smart-cracking, Jimmy-Olsen-type gee-whiz kid." Leonard Freeman took the criticism to heart, and recast the part with James MacArthur for the series.

Answer: He was fired. He never really appreciated his character. He felt Kono was portrayed as a big dumb Hawaiian and that the stereotype was racist. He also felt underutilized. He was fired after a heated argument with the show's publicist regarding his character. It seems there's not much details given regarding the incident or the firing, so it's seems possible he upset the show's producers as well.

No he said something derogatory about one of the Jewish producers that's why he was fired.

Answer: He was asked to speak at a local organization event but the producer insisted that Jack Lord be also there. It was an event in honor of Hawaiians and that set him off. He used a derogatory name for the Jewish producer and that pretty much ended his stint on the show.
