The Initiative - S4-E7

Plot hole: Spike must have had his chip implanted while he was unconcious. If so, how did he attack The Initiative scientists without any pain when he tried to escape? You could argue that they programmed him to be capable of violence within The Initiative only, but why would super smart scientists program an evil being to be able to escape at the first opportunity? Joss Whedon has acknowledged this as an oversight.

Plot hole: Before the opening credits where Buffy stakes the vampire in the cemetery, she stakes him below the left shoulder, above the heart. Yet he still turns to dust and dies.

Plot hole: When Buffy is giving chase to Faith, Faith jumps over a wall. Buffy then looks over the wall to an open field but can't see Faith anywhere. There is no way Faith would have disappeared in that short of time in an open field like that. (00:27:00)

Plot hole: Willow hacks into The Initiative's computer system and discovers air ducts and electrical conduits leading to Adam behind 314, but the room doesn't show up on the schematic. It seems unlikely that nobody who didn't know about Adam would have noticed this and investigated.

Continuity mistake: Willow is turned into a ghost. Throughout the entire episode, she can't touch anything (can't turn the pages of a book, etc.), yet towards the end when she and Giles go to Ethan's shop, and she leaves, you can hear that she opened and closed the door, even though she can walk through walls and can't actually touch the door. The curtain moves when she leaves too. (00:34:05)

Doomed - S4-E11

Question: Specific answer needed, not a general "dialogue is sometimes re-recorded". Near the start of this episode, Buffy's talking to Giles about the significance of the earthquake. Buffy's dialogue sounds completely normal and recorded at the time, but all of Giles' dialogue is dubbed. I can only think he had a sore throat that day or similar, but does anyone know the specific reason he did his later?

Chosen answer: He had larangitis and because of the shooting specifications they had to have him shoot the scene with a bad throat, they then re-recorded his lines and dubbed them. At various points during the scene and indeed the episode you can see him touch his throat or rub it.
