Garma Strikes - S1-E6

Audio problem: The officer is trying to force Bright to send out of the Gundam to break through the enemy forces. Kikka is singing, which makes him lose his temper. You can see though that when she supposedly sings her mouth is instead closed. (00:05:30)

Icelina- Love's Remains - S1-E11

Audio problem: In the throne room of Zeon, Dozle opens the big portal door with one hand, and it makes a creaking sound as if it were a small wooden door of a broom closet. The door itself is closed in the following shots, with no sound heard for it. (00:04:00)

Icelina- Love's Remains - S1-E11

Audio problem: When Char is chased away by Bright, Hayato and Kai shooting at him (even if he's a skilled commando and is running from a couple teenagers), the gunshots of the White Base officers produce 6 detonations in total but many more light blasts. (00:16:00)

Coming Home - S1-E13

Audio problem: The first refugee to greet Amuro has a male voice (original version, but also English dub), but when you look at the figure, it's kinda rounded in the chest and in the next shot when just the legs are in sight, just match the positions of those around Amuro and it's quite clear that 'he' is the one wearing a skirt. Escape from Luna II - S1-E4

Plot hole: White Base and the interior of Luna II withstand without a scratch the explosion of the thermonuclear reactor from the Magellan from just a few meters, while the blast literally vaporizes the Zaku far away from the entrance and threatens to damage the Musai, hundreds of meters away.
