Continuity mistake: Bond arrives in West Berlin to travel to Karl-Marx-Stadt. The Mercedes stops just before Checkpoint Charlie and the other guy gets out of the car. One can see the license plate - which appears to be genuine German (DIN font and spacing etc). When the "same" car later arrives at the Circus the plates are obviously props. The font is off and the plates are obviously fake.

Revealing mistake: The group ambushes a German patrol with a half track and a tank. After the fight is over Dolph sits on the ground leaning to the tracks of the tank. The Lieutenant comes back alone holding his Thompson submachine gun. You can see that the barrel is plugged with an aluminum insert to enable the operation of blank cartridges.

Revealing mistake: In the scene where the boy is shot in the leg and the carried away by an unionist soldier you can see more union soldiers storm past them. The bayonets of the soldiers are obviously made of rubber as they are dangling while the rifles are moved up and down by the running soldiers.

Other mistake: Near the end when the bad guy blows up in the train:The fireball created by the exploding bracelet would be much more suitable to several kilos of high explosive or a fuel explosion. I could NEVER been created by the amount of explosives stored in a bracelet (see as well Mythbusters, the James Bond episode with the exploding pen).

Other mistake: After stopped by the pirates and shooting them, Rambo accelerates his own boat again. The boat is driven by a rear mounted engine with a long shaft, with the screw 2 meters behind the boat. But turbulent water is exiting directly under the boat, which would suggest that the boat is equipped with a regular inboard motor, with the propeller under the stern of the boat.

Factual error: At the scene in the church, there are lots of security personnel standing around wearing steel helmets. The helmet type seems to be an East German M56 helmet (standard for the East German Armed forces until reunification). There is no reason why a Russian security force would have the M56 helmet. (Only if the makers of the movie where able to purchase a lot cheaply :).)
