Question: John puts a grenade on a fork lift and wheels it over to the bold, shirtless, muscular guy shooting at them. It explodes big and almost water like. What kind of bomb/grenade was that? (01:21:10)
Chosen answer: It was a normal hand grenade but they shot a few gas lines earlier in the shootout, which caused your mentioned "water-effect".

Plot hole: When John has Yuri pinned to the uranium boxes, Yuri reaches into his coat and turns on his walkie talkie so his daughter and minions can hear. John McClane doesn't notice and he is right there. There are two things wrong with this. We hear the beep the radio makes when it is turned on, and it was loud enough that John and Jack would have easily noticed. But even if they didn't hear it, there is another issue. John is a New York cop, or even a detective. As such, he would have extensive training for dealing with armed men, and he would have noticed Yuri reaching into his coat. Even if he didn't hear the radio beep, he would have stopped Yuri's hand or done something, assuming Yuri was reaching for a gun. (01:18:15)

Continuity mistake: Just after John asks Jack if he is "gonna cry a little" before he pulls the bolt out of him, Jack leans his head back. The next shot, Jack's head is down. Throughout this scene, his head keeps changing position between shots. Also noticable right after John says "You shot?" (00:58:25)

Other mistake: When John and Jack are in the ball room, the chopper circles back and begins shooting through the building at them. It is shooting into the far right window straight forward. John and Jack get up and take off running. As they do, you can see the bullets' line of fire moving with them through the building as the chopper tries to hit them. But it cuts to an outside view and the chopper has not moved or turned yet from that spot, nor are the guns turning. It is visibly still shooting forward at the same spot. The line of fire follows Jack and John some more, but the outside view shows the chopper just then start moving to begin following their movement. (00:52:40)

Continuity mistake: When John and Yuri are standing in front of the crane and talking about their children, John's arms go from crossed to by his side, back to crossed a couple times between shots. (00:39:35)

Continuity mistake: When we first see John, he is in the police shooting range. He has on protective head phones which he takes off and sets on the table in front of him as the other cop comes in. He and this cop talk about Jack, and when they finish, they shake hands. The mid range shot of them shaking hands shows the head phones still on the table. It then cuts to a far shot and now the head phones are gone while the two finish the hand shake. Careful which table you are looking at, as the angle makes it confusing. The head phones were on the table with the red thing on its left side. (00:06:50)

Question: Who is the guy suposed to be that Jack kills in the beginning at the night club? (00:04:50)
Answer: His name was Anton. He was an associate of Viktor Chagarin. Jack, although lying, said Yuri Komarov sends his regards. Chagarin and Komarov were former partners and Chagarin was the one who got Komarov put in jail. By Jack saying Komarov sent him, it made it seem like Komarov was out for revenge on Chagarin (by attacking his associates). The courts offered to reduce Jack's sentence for testifying against Komarov, which allowed Jack to get close to him and help him escape.
Answer: Just a nobody. Jack had to kill someone so that he would be arrested and that was his way of getting to the old guy (I forget his name). Additionally the guy in the nightclub was probably an enemy of the other Russian guy and it was a way of getting rid of him.

Question: Is John McClane ambidextrous? I noticed in several scenes, he'd alternate between using and holding his gun with his right and left hand.
Chosen answer: No. Bruce Willis (and thus, John McClane) is left handed. But guns are mostly designed for right handed people, so when he isn't holding a gun modified for him, it's easier to use it with his right hand.

Revealing mistake: When the armored truck flies through the air and smashes into a concrete pipe that is being hauled by a yellow truck, there is a shot from in front of the yellow truck. You can see there is no driver. (00:28:55)

Continuity mistake: In the car chase, the blue van is hit and the left side headlight dislodges and is dangling down for most of the chase. But in one far shot where the van is taking a turn, it is suddenly switched to the right headlight that's dangling. (00:23:00)

Continuity mistake: Just after John says "Jack! I'm not done talking to ya!" It shows the blue van backing up and turning around. The first shot shows its headlights turned on, but the next shot they are off. It happens again later when the van is spun and pinned to the front of the armoured truck. (00:22:20 - 00:27:20)

Visible crew/equipment: After John stops the blue van and tries to confront Jack, Yuri takes it upon himself to get out. As he opens the door of the van, a camera and some crew equipment is reflected in the door's window for a brief moment. (00:19:05)

Continuity mistake: The taxi driver looks back at John and says he doesn't have to pay because he let him sing. The shot from in front shows the steering wheel is clear. But the shot changes to behind John and suddenly the driver's arm is on top of the steering wheel and across it. (00:12:00)

Continuity mistake: In the taxi, the driver says the line "I sing all the time". As he is saying this, there is a shot change. The shot from in front shows his right hand is barely visible next to the steering wheel and semi low. But the shot from behind shows his hand higher with thumb sticking up above the steering wheel. Continuity mistake: As Lucy tells John that it's not his fault, the angle showing her shows John with his mouth open. It cuts to another angle and close up of John and suddenly his mouth is closed. (00:07:10)

Continuity mistake: When we first see John in the shooting range, the other cop brings him news of Jack. John asks if he is in the hospital or morgue. The other guy says "worse". The shot from behind John shows he is looking right at the other guy. But then the shot changes to the other perspective and suddenly John is looking down a little and in a different direction.
