Revealing mistake: In driving scenes, the driver of the car usually has very over exaggerated movements of his or her hands on the steering wheel. When in reality you're not moving it that noticeably except a few micro corrections every few seconds or taking turns.

Deliberate mistake: Especially in action movies and westerns, people in gun scenes often go through way more ammo in their weapons than possible and never reload. Especially the heroes.

Deliberate mistake: Characters in super hero movies commonly wearing only a small mask over their eyes which somehow keeps people from identifying them. Despite the fact that most of their facial features and hair styles are visible and recognizable. Great examples being Robin, Green Lantern, The Lone Ranger, and The Incredibles. And of course Superman just takes off his glasses.

Suggested correction: They use this to obscure people's faces in newspapers, etc., so maybe it actually works well enough for the average person. I have a mild form of face blindness, where I recognize people mostly by details of their faces (e.g. the mouth shape), not the face as a whole, so I always found that ridiculous, but it seems to work on others if they are not familiar with the person.

The way this is worded is a bit confusing. But having it done in newspapers, as opposed to a person being in person around other people or on film while moving, is an entirely different matter.

Factual error: Hearing a dial tone after someone hangs up a phone - that wouldn't be heard unless you hang up and pick it back up yourself.
