Continuity mistake: The warden attempts to trap Ricky by pushing a table at him. On the table there is a tray filled with meat from when the warden forced an inmate's arm into a meat grinder earlier in the scene. In the shot where he first pushes the table, the meat in the tray is towards the right hand corner of the tray. Ricky then jumps over the table and the meat is now towards the left hand side. Revealing mistake: When the warden has finished shoving the inmate's arm into the meat grinder, there is a shot of the warden pulling the inmate away from the grinder and showing him the tray of meat made from his arm. In the shot, we can see that the inmate's face has been covered in white powder to make him appear as if his face has gone pale due to the blood loss, but it makes his face look completely unnatural, and not at all how a human face would ever appear in a situation like that. Revealing mistake: Whilst the cell is filling with cement, Ricky runs over to the steel door and bursts through it to escape. In the shot of him bursting through the door, we can see that the interior of the door is extremely thin and is barely a millimeter deep, revealing that it is a fake prop door and not steel. Revealing mistake: In the shot where Ricky punches the prison wall, a split-second before Ricky punches it, a faint outline can be seen to the right of him that gets blasted out of the wall a second later, revealing where the wall was filled in with fake debris and later plastered over. (01:29:59)

Other mistake: Whilst Ricky is fighting Wong Chun, White God attempts to sneak away by getting into one of the dumbwaiters, but the warden shoots him and he explodes. The bullet is shown hitting him in the face, but given the position of White God's body and where the warden was shooting, this would be impossible. When White God crawls into the dumbwaiter, the pantry door is shown closing behind him, so the bullet would have been blocked by the door and would have made it impossible to hit him in the face. Continuity mistake: When the warden is kicked across the kitchen by Ricky, he lands on a sink. On the floor beneath the kitchen sink, a couple of tiles that were stuck to the wall fall off it. We are then shown a closer shot of the same area, where the tiles that just fell off the wall are now back on the wall, and two entirely different tiles have now fallen off. (01:26:31)

Revealing mistake: When Ricky jumps over the table after the warden pushes it at him, there is a shot of Ricky kicking the warden with his foot whilst in mid-air. In this shot, we can see that Ricky's trousers are being pulled upwards, revealing that there is a wire attached to his trousers that is pulling him into the air. (01:26:30)

Revealing mistake: When the assistant warden explodes after his body starts inflating due to the warden shooting him with the elephant bullets, in the close-up shot of his head exploding, we can see that his head has been replaced with a fake dummy head. (01:23:18)

Revealing mistake: Just after the warden has shot the assistant warden with the bullet made for shooting elephants, there is a shot of the assistant warden's body inflating. In the shot, we can see that really bad prosthetics have been added to his forehead, cheeks, nose and chin to make it appear as if his head is inflating. It is extremely noticeable where the prosthetics end and where his real face begins, making the whole effect appear completely unnatural. (01:23:12)

Revealing mistake: When the prison wall begins to break apart after Ricky punches it, in the wide shot where the debris from the punch is shown falling to the ground, multiple wires can be seen pulling the chunks of debris from the wall. (01:30:04)

Continuity mistake: When Ricky begins to charge towards the wall to punch it down, his right fist is shown to have blood on it. When the camera cuts to angle behind Ricky, the blood on his right fist has suddenly disappeared. (01:29:55)

Continuity mistake: When Ricky is trying to force the warden's body into the meat grinder, as Ricky begins pushing his head down, the warden is positioned directly in front of Ricky. When the camera changes angles, the warden has suddenly rotated by 90°. The camera changes angles again and the warden is suddenly facing Ricky directly, then in the next shot, the warden has suddenly rotated 90° again. (01:28:53)

Continuity mistake: When the warden is thrown across the kitchen and into the meat grinder by Ricky, in one shot, he is shown being thrown across the room from left to right. When he lands in the meat grinder in the next shot, he is suddenly falling downwards. (01:28:37)

Other mistake: Just after the warden tells Ricky they are trained in the same kung fu style, Ricky thinks to himself for a moment. The subtitles read 'If your opponent is strong, use your wits to beat him', but before the start of the line, before the word "If", a randomly placed comma can be seen at the start of the line. There is no reason for this comma to be here since there are no words shown before it. Clearly this was an unintentional error made when the subtitles were being typed out. (This depends on which format and which studio distribution you are watching, but it can definitely be seen on the Region B Blu-Ray distributed by MediumRare). (01:28:10)

Continuity mistake: When Ricky is sitting on the ground after being thrown to the ground by the warden, whilst the warden is telling him they've got the same kung fu style, Ricky has both his arms placed firmly on the ground. The camera changes angles and suddenly, his left arm is raised off the ground. (01:28:05)

Revealing mistake: Just after the warden throws Ricky to the ground, when he finishes saying "Your strength doesn't even come close to mine", he starts doing an evil laugh and here, we can see the actor's real tongue and teeth behind the fake prosthetic suit that he is wearing. (01:28:02)

Continuity mistake: In the final fight scene, there is a moment where Ricky punches straight through the warden's stomach and punches out through the other side. The shot then cuts to the warden for a second, but when the shot cuts again, Ricky's arm has suddenly been completely removed from the warden's stomach. No way could Ricky have removed his entire arm from the warden's stomach that quickly. (01:27:33)

Revealing mistake: When Ricky attempts to fight the muscular warden, he punches the warden in the stomach and the warden then punches him into a support column. Just before Ricky crashes into the column, a wire can be seen attached to him, pulling him backwards towards the column. (01:27:20)
