Other mistake: When Suzanne is in the trap there is one point where we see 36 seconds remaining on the clock. If you listen carefully you can continue to hear the clock tick down once per second, even offscreen, and you can count about 45 ticks before Suzanne is killed. (00:44:45)
Continuity mistake: When Gibson asks Jill if she knows who the man in the picture is, he is holding the picture in his left hand. When the camera cuts it is now in his right hand. He's also reaching between different bars. (00:48:40)
Continuity mistake: When we see Cale's trap, he has got sweat stains on his shirt, in the shot from the floor, there is no sweat stains on his shirt. (00:50:30)

Revealing mistake: If you look very carefully when Gibson is machine gunned down, you can see the blood packs in his jacket. Slow-Mo may help but is not essential. (01:07:15)
Visible crew/equipment: When Hoffman runs to hide from the second coroner, he goes right up close to the camera. When he does, you can see the camera's shadow on his face. (01:08:00)
