Revealing mistake: When Trinity's being chased by the cops in the first scene and she makes her first jump from one roof to another when being chased by the cops. One of the cops that is chasing her doesn't quite make a complete jump to the other roof. During his failed attempt when he hits the lip of the roof of the adjacent building it makes the whole building shake. (00:04:00)
Revealing mistake: When Neo breaks free from the goo-pod and slides down the tunnel into the sewer, he falls into the 'lake'. But if you look close you can see that when he falls he's wearing a nappy of sorts, covering his modesty. (00:33:00)
Revealing mistake: When Neo is fighting Morpheus in the training room and Neo runs up the wall, you can see Keanu Reeves is holding onto the wires attached to the safety belt around his waist. (00:51:25)
Revealing mistake: When it is Neo's turn to make the Jump he takes a run up, and the camera changes to a brief sideshot of him running across the top of the building with the sky behind him. There is a big fuzzy blobby outline around him which should have been edited out. (00:52:20)
Revealing mistake: When Neo is being trained in the "jump program" Neo jumps and fails to make it across. When he is closer to the camera, the wind blows up his shirt and you can see the black safety harness on his abdomen. (Easier to view in slow motion). (00:54:35)
Revealing mistake: When Trinity, Neo, Switch, and Apoc are in the wall, after they plummet down, land, and start shooting everyone, watch Trinity as she falls. She is obviously attached to a wire and gets jolted when she falls. It looks as if the wire didn't have enough slack. (01:19:55)
Revealing mistake: When Neo shoots the guy with the newspaper at the beginning of the lobby scene it is obviously a prop. It is very thick paper stuck together almost like cardboard because the individual pages are not seen where it rips. Also it would be near impossible to split a newspaper like that even though it does take a few bullets. (01:37:15)
Revealing mistake: When Trinity throws the knife into the guard's face on the rooftop it quivers like rubber when the guy starts to fall. (01:41:35)
Revealing mistake: In the government lobby scene, when Trinity kicks the shotgun out of the guard's hands, watch closely, the man actually throws the shotgun over his own shoulder. It's very well timed, but if you watch closely you should be able to spot it. (01:42:30)
Revealing mistake: During the rooftop rescue, when Morpheus is hanging from Neo's arm (who is hanging from the helicopter), a safety wire is visible running out of Morpheus' sleeve and into Neo's hand. (01:45:48)
Revealing mistake: When Agent Smith is fighting with Neo in the subway, Neo's back is against a pillar. The area of the pillar which Agent Smith punches away with the first punch is visible before Neo ducks, and then he punches it away. You have to look carefully, because it only appears for a very short period of time. (01:50:35)
Revealing mistake: During the fight scene between Neo and the agent in the subway station, after Neo incapacitates the agent and flips out of the way of the oncoming train, you can see where the wire harness connected to his back. You can also see one of the wires visible by his wrist. (01:57:38)
Revealing mistake: At the end of the movie when Neo bursts into Room 303 and Agent Smith is there, watch the second shot. The overhead view shows a blank-firing shell ejecting (crimped end). (01:57:45)
Revealing mistake: When Neo and Agent Smith are fighting on the subway tracks, Neo leaps up to avoid being hit by the train. While the shot is in slow motion, you can actually see the wire supporting Keanu Reeves (just look at Neo's hands grabbing them). The wire becomes momentarily visible right when he is reaching the highest point of this jump. (01:58:15)
Revealing mistake: When Neo stops the bullets flying at him at the end you can see that the bullets don't have any ridges from the rifling. (02:05:15)
Revealing mistake: Near the start, when Trinity runs away from Agent Smith, jumps off the building, crashes through the window, rolls down the stairs and gets out two guns, when she jumps from the building, it is obviously a stunt double.
Revealing mistake: After Morpheus and Neo are loaded into the "Jump Program" you can see the Green Screen on the top right corner of the scene. This appears right before Morpheus jumps across to the other building.
Revealing mistake: In the scene when Trinity makes it back, after Morpheus is taken, she hugs Tank. When she hugs him, you can see that she doesn't have a jack in the back of her head.
Suggested correction: In fact, she has the plug.
Revealing mistake: When Neo stops the bullets being shot at him in mid air and they fall, you can see that there are not grooves in the bullets. As a bullet passes through a barrel, it picks up the impressions of the lands and grooves of the barrel. Since the Matrix is a simulation of our world, including the involved physics, the lack of rifling grooves is an oversight by the special effects team.
