Revealing mistake: When the sorcerer is about to show Toulon his magic for the first time, Toulon picks up one of the puppets. A few moments later, the sorcerer tells him to put it back down, so he does. Both times this happens, you can tell they used the exact same cutaway shot of Toulon picking up the puppet. They simply reversed the footage in the second take to make it look like he’s putting the puppet down. The distinct movement gives it away.
Revealing mistake: In the widescreen Blu-ray version, when Pinhead is brought to life, you can see the side of the puppeteer's hand tilting him up. The movie was originally shot for home video with the intent of being cropped down to fullscreen. The Blu-ray version presents the original uncropped widescreen version, unfortunately causing this mistake to be caught in shot.
