Beethoven (Ed Harris) dies at the beginning of the film and Anna (Diane Kruger) tells him that the "Grosse Fugue" is wonderful and that she tried to picture it like he would. Anna helps Beethoven in his final days composing the "Grosse Fugue" which he writes as a "thank you" to God for giving him the talent to make music; she's the only one who supports his idea and everyone else think he's gone mad. Beethoven dies on March 26, 1827 and his "Grosse Fugue" inspires future generations of composers...

Answer: It's a fictional version of Beethoven. Artistic license is used to enhance the drama by embellishing Beethoven's personality, likely to show his anger, frustration, and despondency over becoming deaf. Like many similar biographical movies, it is not meant to be an accurate portrait.
