Continuity mistake: Just before Melvin has sex with Sarah, Sarah's red dress straps fall down off of her shoulders, yet are back up in the next shot.

Continuity mistake: When Melvin falls into the toxic waste, he is wearing tights and a tutu, but when his stunt double runs up the street in flames, he's obviously wearing trousers and a shirt.

Continuity mistake: When Melvin is putting the cream and ice cream on one of the robber's face, the bowls and bottles, and the (ice) cream on the robbers face all keep changing position.

Visible crew/equipment: When the car goes off the cliff towards the end, you can see crew members watching and a red van with crew members in it.

Continuity mistake: When Toxie and Sarah are having sex, they're in the missionary position, and she has her legs wrapped around him. She's wearing a pair of red high heels which she then kicks off, but in the following shot the shoes are back on her feet.

Audio problem: When Toxie is chasing Julie, Julie runs into a room, locks the door and puts a plank of wood to secure the door. As she does, she cries and whimpers, yet her mouth doesn't move.

In this film the title character is never referred to as "The Toxic Avenger"; he is only referred to as "Monster Hero." In his first book, "All I Needed To Know About Filmmaking I Learned From The Toxic Avenger," the director Lloyd Kaufman revealed that the reason for this was because the working title of the movie was "The Monster Hero," and that the change to the title "The Toxic Avenger" only occurred at the "last minute." The character was consistently referred to as Toxic Avenger, or Toxie, throughout this movie's three sequels: "The Toxic Avenger II," "The Toxic Avenger III: The Last Temptation of Toxie," and "The Toxic Avenger IV: Citizen Toxie."
