The Lord of the Rings: The Return of the King

Factual error: After Gollum attacks Frodo and Sam on Mount Doom, Sam fights Gollum off with his sword. He then turns around and calls out for Frodo. There is a brief medium shot of Frodo scrambling up the mountain (at about 01:20:35 on disk 2 of special edition). If you look carefully in the background, you can see snow and running water in the background. This is common on Mt. Ruapehu, where the scene was shot, but a bit out of place on Mt. Doom.

Continuity mistake: After Denethor drops a torch on himself and Faramir, Gandalf takes a spear off a guard and as Shadowfax runs towards Denethor, the guard he took the spear from starts to walk. In the next shot the guard is standing completely still.

Audio problem: When Gandalf says to Pippin mercenaries are coming from the coast, a captain can be seen on one of the ships. The captain says "Come on", but his mouth doesn't match what he is saying. (Timecode from extended DVD).

Revealing mistake: Just after Aragorn says "For Frodo", he starts charging at the Orc army. After that Merry and Pippin start charging too followed by everyone else. When everyone else is running past Merry and Pippin, look closely at them. You can see it is not Merry and Pippin but their scale doubles.

Continuity mistake: In the extended edition, when the Black Gate opens the first time, the Mouth Of Sauron comes out and shows them Frodo's elvish vest to make them think he is dead. Aragorn then kills him on the spot. Yet when the Black Gate opens the second time, there is no sign of the Mouth Of Sauron's dead body,.

Revealing mistake: When Frodo is unconscious due to the Shelob sting, just after Sam says "Dead" and the shot cuts to Frodo, you can see his eye flinch even though he is supposed to be unconscious.
