Visible crew/equipment: When the two characters are talking in the car going down the road, you can see the reflection of some crew members in the sunglasses. (00:13:00)
Continuity mistake: When Hunt and Warren are in the elevator, Warren is wearing normal clothes. When the storm into the O.R. Warren is suddenly wearing scrubs. There is no way he could have changed into scrubs fast enough to get to the room before they started cutting the bars off the guy on the table.
Continuity mistake: When Addison first comes to Seattle, she mentions that she likes Derek's hair. She says it's longer than usual, looks like Russell Crowe. In the flashback of the night he left his hair long like it always is. Actually it seems a little longer in the past than current.
Continuity mistake: When the episode ends and Jo and Alex are throwing balls at the wall, the position of the bag below changes between shots.
Revealing mistake: When Alex is talking to Rebecca Pope at the end of the episode, we hear Alex say "Now I know more about you than my friends." Watch Rebecca's left eye on the right if the screen when she says, "That's pathetic." Someone did a bad job of editing the CGI and we see a phantom eye over her left eyebrow.
Other mistake: There are two mistakes when Grey is at the computer trying to figure out why her patient is having seizures. The camera pans out showing Bailey walking up - if you look at the computer it shows a login screen, but when Grey shows Bailey the screen it changes. Next one is when Grey blows up the pic of the taratoma - when they leave the picture on the computer it's actually a brain.
The Self Destruct Button - S1-E7
Continuity mistake: While Meredith and Izzie are in the kitchen discussing Derek, Izzie sets the freshly frosted whole chocolate cake on the counter, but when she takes a seat in the closeup there's a slice of cake already on the plate, and in following closeups the cake is either whole again or back with the cut slice. (00:39:45)
Continuity mistake: Izzy tells the male patient she is a good doctor even though she was a model. Her hair is parted to the left and in the next shot it is parted to the right.
Continuity mistake: Arizona is wearing fitting black slacks and black heels and has two legs (not a prosthetic on the left). (00:37:45)
Other mistake: The exact same shot of Derek's radio tuner is used twice-once when he calls Amelia and again when he gets in the car after helping at the accident. Both times, the clock says 7:45PM, but Alana (the AP class taking, cello playing student) says that she's supposed to be at school, implying it's sometime between 8AM-ish and 3PM-ish. Also, the sun sets around 7:30PM in Seattle on March 26, but it is obviously day time for the duration of this scene so there's no way it could be 7:45PM in either of the shots in which the clock is shown. (00:03:30 - 00:18:40)
Other mistake: When Derek has finished showing Callie and Arizona the picture of Cristina getting her nomination, he leaves to take the kids into what we are supposed to believe is daycare...however the sign on the door reveals it is a dispensary. (00:02:30)
Visible crew/equipment: After Meredith discovers Derek visiting her mother at the nursing home, when it cuts to Miranda in the OR the T-mark can be seen on the floor at Cristina's feet. (00:26:40)
Owner of a Lonely Heart - S2-E11
Visible crew/equipment: In the NICU, when George starts boasting that he has the best quint, in the overhead wideshot we can see the T-mark (greenish yellow) on the floor near Kate, moments before Karev walks in and stands there. It's visible again just as Karev walks away. (00:01:05)
