Question: How can Hershel be driving without his lower right leg?

Chosen answer: Since the show is set in America, it's likely to be an automatic gear shift, so while it would be uncomfortable and take a lot of getting used to, Hershel would be able to drive with his left foot working both the gas and the brake.
